The film is a graphic depiction of the war atrocities committed by the Japanese at Unit 731, the secret biological weapons experimentation unit of the Imperial Japanese Army during World War II. The film details the various cruel medical experiments Unit 731 inflicted upon the Chinese and Soviet prisoners towards the end of the war.
This is the first film to be rated "III" (equivalent to the US rating NC-17) in Hong Kong.
Synopsis
The film opens with the passage "Friendship is friendship; history is history." Mou said of the quote, "that's the argument [I had] with the [Chinese] government, because the government says 'no, we have a friendship with the Japanese', I said 'well, you can talk about your friendship, I am talking about history.'"Actors

During the prologue, a squad of British soldiers is seen assaulting a German trench at night. Sergeant David Tate (Hugo Speer) is caught in the barbed wire and calls out to Private Charlie Shakespeare (Jamie Bell), a 16-year-old member of the squad, to rescue him from the entanglement. Shakespeare, however, has completely broken down from fear due to the chaos around him, and runs off into the fighting, only to be seemingly buried by shellfire moments later.

In 1943 two Belarusian boys are digging in a sand field looking for abandoned rifles in order to join the Soviet partisan forces. Yustin, an old man, warns them not to dig (using sarcasm and reverse psychology). One of the boys, Flyora (Aleksei Kravchenko), finds an SVT-40 rifle. The next day partisans arrive at his house and take Flyora with them, to the dismay of Flyora's mother. She fears that the loss of her son, like his father before him, will lessen her and her daughters' chances of survival.

The movie does not have a central storyline, though it follows individual members of a Chinese family during the Nanking Massacre. The movie begins after the Japanese have captured the city and begin a "cleanup" phase. Homes are broken into and burned, the residents killed and raped. Bowing to international pressure, the Japanese government has agreed to establish refugee safety zones, only to violate the agreement at their whim, knowing that there is no force that can really prevent them from doing so. As a result, they regularly enter the refugee camps, kidnapping women and executing Chinese policemen and suspected soldiers. Several foreign residents try to defend the refugees, and while they carry weight due to their political ties, they ultimately cannot stand against the armed troops. Some of the foreigners depicted in the film include the German businessman John Rabe and American missionary Minnie Vautrin.

A handbill posted on a burnt tree, dated 1862, announces that anyone interfering with bridges, railroads or tunnels will be summarily executed. A bearded Civil War civilian prisoner, Peyton Farquhar, is readied for death by hanging from a rural railroad bridge; Union troops carry out the preparations with slow solemnity. In a Provençal village in the south of France, the villagers welcome the declaration of war with Germany in 1914 and flock to enlist. Among them is François Laurin, a man of jealous and violent temperament, who is married to Édith, the daughter of an upright veteran soldier Maria Lazare. François suspects, correctly, that Édith is conducting an affair with the poet Jean Diaz who lives in the village with his mother, and he sends Édith to stay with his parents in Lorraine - where she is subsequently captured and raped by German soldiers. François and Jean find themselves serving in the same battalion at the front, where the initial tensions between them give way to a close friendship that acknowledges that they both love Édith.
