Sally Hardesty (Marilyn Burns) and her paraplegic brother, Franklin (Paul A. Partain), travel with three friends, Jerry (Allen Danziger), Kirk (William Vail), and Pam (Teri McMinn), to visit the grave of the Hardestys' grandfather to investigate reports of vandalism and grave robbing. Afterwards they decide to visit the old Hardesty family homestead. Along the way they pick up a hitchhiker (Edwin Neal) who talks about his family who worked at the old slaughterhouse. He borrows Franklin's pocket-knife and cuts himself, then takes a Polaroid picture of the others and demands money for it. When they refuse to pay, he burns the photo and slashes Franklin's arm with a straight razor. The group forces him out of the van and drive on. They stop at a gas station to refuel, but the proprietor (Jim Siedow) tells them that the pumps are empty.

High school student Alex Browning boards Volée Airlines Flight 180 with his classmates for their senior trip to Paris, France. Before take-off, Alex has a premonition that the plane will suffer a catastrophic engine failure, causing the plane to explode in mid-air, and killing everyone on board. When the events from his vision begin to repeat themselves in reality, he panics and a fight breaks out between Alex and his rival, Carter Horton. This leads to several passengers being removed from the plane, including Alex; Carter; Alex's best friend, Tod Waggner; Carter's girlfriend, Terry Chaney; teacher Valerie Lewton; and students Billy Hitchcock and Clear Rivers. Sam Lawton is on his way to a company retreat with his colleagues. While their bus crosses the North Bay Bridge, Sam has a premonition that the bridge will collapse, killing him and everyone on it, except for his ex-girlfriend Molly Harper, who he gets across the bridge safely. In a panic, he persuades Molly; his friends Nathan Sears and Peter Friedkin; Peter's girlfriend Candice Hooper; his boss Dennis Lapman; and co-workers Olivia Castle and Isaac Palmer to leave the bridge before it collapses, killing the remaining people on it. FBI agent Jim Block does not believe that Sam is responsible for the bridge collapse, but promises to keep his eye on him. At the memorial service, coroner William Bludworth mysteriously tells the survivors that "Death doesn't like to be cheated" and warns them to be careful. Believing this to be nonsense, they ignore his warnings and leave.

The father of New Orleans schoolteacher Annie Tarrant (Rowan) was murdered in a Candyman-like fashion some years prior. When Professor Philip Purcell is murdered in a bathroom by Candyman after presenting the legend at a book signing and calling him forth, Annie's brother is accused of the murder (since his furious public confrontation of Purcell over the subject) and one of her students starts to see the Candyman. Several years after the events of the second film, the ghostly serial killer returns once again from beyond the grave; this time, during the eve of Day of the Dead, to haunt a Los Angeles art gallery owner named Caroline McKeever, a distant relative of the Candyman (and also Annie Tarrant's daughter) in order for him to claim her soul so she will be next to him. In the meantime, the Candyman goes about killing all those associated with Caroline (starting with artist Miguel Velasco, her lover, and following with her roommate Tamara) in his usual gory ways with his hook and making it appear to the authorities that Caroline is the one responsible for the killings. Particularly when seasoned police detective (and closet prejudice of most minorities) L.V. Sacco dies, which not only brings the whole local police department down on her head; but puts her in the firing line of Sacco's equally bigoted partner Lt. Det. Samuel Deacon Kraft who has no intention of bringing her in alive.

Picking up immediately where the first film ended, Marybeth (Danielle Harris) is attacked by Victor Crowley (Kane Hodder), but manages to escape. In 1944 in the Republic of Salò, the Fascist-occupied portion of Italy, four wealthy men of power, the Duke, the Bishop, the Magistrate, and the President, agree to marry each other's daughters as the first step in a debauched ritual. They recruit four teenage boys to act as guards and four young soldiers (called "studs", "cockmongers", or "fuckers"), who are chosen because of their big penises. They then kidnap nine young men and nine young women and take them to a palace near Salò. Accompanying them are four middle-aged prostitutes, also collaborators, who recount arousing stories for the men, who sadistically exploit their victims.
