A group of college friends--Harvey, Nikki, Rob, Skip, Nan, Chaz, Kit and Arch--gather to celebrate Spring Break by spending the weekend at the island mansion of their friend Muffy St. John, on the weekend leading up to April Fools' Day. The tone is set almost immediately, with Muffy preparing details around the house, and finds an old jack-in-the-box she remembers (in flashback). Paul Toombes (Vincent Price) is a successful horror actor whose trademark role was Dr. Death, a skull-faced sadist. During a party in Hollywood showing off his fifth Dr. Death film, he announces his engagement to Ellen Mason (Julie Crosthwait), who gives him an engraved watch as an engagement gift; later that evening, however, adult film producer Oliver Quayle (Robert Quarry) reveals Ellen had worked for him previously. Distraught at Toombes' reaction, Ellen returns to her room, where a masked man in dark garb, similar to Dr. Death's attire, approaches her with a knife. An apologetic Toombes comes in shortly after, only for her decapitated head to fall from her shoulders. Though he is acquitted of the crime, Toombes' career is destroyed as he spends several years in a mental hospital, where even he is not sure whether he killed Ellen or not.

The plot revolves around the return of Brian Woods (Leitch), a "problem teen." He has just been released from a mental hospital; he was admitted after the suspicious death of his father, who crashed and died while driving a car with cut brakes. He falls in love with classmate Paula Carson (Jill Schoelen), but the local basketball star Dwight Ingalls (Brad Pitt) is already Paula's boyfriend. Meanwhile, the lecherous school principal also seeks Paula's affections. Then horrible murders start happening with no one certain of the identity of the culprit.
