It is a sequel to Barker's 2008 film Outpost. The film was later followed by Outpost: Rise of the Spetsnaz, the third entry in the series.
Synopsis
Nazi-hunter Lena (Catherine Steadman) is on the trail of a notorious war-criminal scientist, Klausener, who at the close of World War II had begun trials of a frightening new technology that can create an immortal army. Lena's hunt leads her to a war zone in Eastern Europe (although the exact location is never mentioned, maps clearly show former Yugoslavia). There she runs into an acquaintance, a physicist, Wallace (Richard Coyle). He encourages her to stop her search because he knows what is coming and she refuses. So, pooling resources, they end up helping a professional military unit they meet take on the advancing army, the product of Klausener's experiments, a battalion of zombie Nazi Storm Troopers. Lena, Wallace, and the unit aim to identify the source of the evil army and prevent a Fourth Reich.Actors

In a seedy bar in a town ravaged by war, scientist and businessman Hunt (Julian Wadham) hires former Royal Marine turned mercenary D.C. In rural Texas, go-go dancer Cherry Darling (Rose McGowan) decides to quit her low-paying job and find another occupation. She runs into her mysterious ex-boyfriend El Wray (Freddy Rodriguez) at the Bone Shack, a BBQ restaurant owned by J.T. Hague (Jeff Fahey) and his sheriff brother (Michael Biehn). Meanwhile, a group of officials at a nearby US military base, led by the demented Lt. Muldoon (Bruce Willis), are making a business transaction with a chemical engineer named Abby (Naveen Andrews) for mass quantities of a deadly biochemical agent known as DC2 (codename "Project Terror"). When Muldoon learns that Abby has an extra supply on hand, he attempts to take Abby hostage, and Abby intentionally releases the gas into the air. The gas reaches the town and turns most of its residents into deformed bloodthirsty psychopaths, referred to as "sickos" by the surviving humans. The infected townspeople are treated by the sinister Dr. William Block (Josh Brolin) and his unhappy, unfaithful bisexual anesthesiologist wife Dakota (Marley Shelton) at a local hospital.

A group of scientists are working on a serum called Death One, which reanimates the dead. When Dr. Alan Holder (Robert Marius) and his assistant Norma experiment on a human corpse, the corpse becomes a zombie, prompting Dr. Holder to resign from the project. As he prepares to surrender the serum to waiting military officers, a small group of criminals ambush the center; all but one are killed, and the surviving criminal manages to abscond with Death One. During a chase, the container with the serum is shot out of his hand and breaks; as he tries to pick it back up, he accidentally touches the serum. He flees to the Sweet River Resort and settles in Room 4. General Morton (Mike Monty) promises Dr. Holder that he and his men will capture the criminal. By the time they catch up to him, however, he has already succumbed to the disease, but not before infecting a bellhop and killing a maid before finally cutting off his own hand in a failed attempt to stop the spread of the infection. General Morton orders the patrons and staff killed and buried in a mass grave, and condemns the resort; the criminal's remains are delivered to him and his right-hand men, Tracey (Bruno Mattei) and Cheney (Claudio Fragasso), and burned. As Dr. Holder and Norma fear, the ashes quickly infect the air, killing an entire flock of birds passing by.

A family of four (Shigeo, Yasuko, Asami and Ryota) on a road trip has their idyllic holiday turned into a nightmare with a bizarre series of events. They stop their car after accidentally hitting a man dressed in white (Aihara) but when he stands up brandishing a gun, another man, Jirō, shoots Aihara. Jirō and his manic girlfriend Lisa then hijack the family's car, holding the father at gun point. When one of the tires is punctured, Jirō tells the father, Shigeo, that they are in the land of the undead and that the father must go off to find help, leaving his family with the captors. When Shigeo finds a ghostly cemetery he starts digging until he finds a blade which he then uses to decapitate himself, with the resulting fountain of blood spraying an ancient tomb. Some time later an eerie samurai warrior appears from the tomb and begins to hunt the family and the captors. The fugitives think to find shelter in an abandoned building but more horrors and surprises await.

On a routine flight from Los Angeles to Paris, a renegade group of scientists has smuggled aboard a secret container holding a fellow scientist infected with a deadly genetically engineered virus which reanimates the dead. The virus is a variant of the virus malaria created by three scientists. They discovered and manufactured the virus with the intent of turning it into a biological weapon. Their goal was to produce soldiers who could continue fighting, even while mortally wounded. The virus is transmitted through bodily fluids. In 1985 in Yugoslavia, a mysterious plague infects & kills a group of archeologists. Police & military secure the area before one of the dead workers reanimates as a zombie & attacks a detective & a fellow officer. In the present day, a military train arrives with a chemical cargo at a train station. A supposed military exercise, communications are cut off & a professor misses his train at the station due to the protocol. A group of homeless thugs also arrive & harass a police officer by intimidating him & stealing his gun. During a brief fight in which the officer attempts to retrieve his gun from the thugs, the gun goes off, injuring a guard securing the military cargo & accidentally puncturing a gas line from the cargo, releasing the toxic chemicals inside. Only the professor is unharmed, having been given a gas mask seconds before he could've gotten infected, & escapes the train station. The gas spreads throughout town infecting many people. Meanwhile an unknown prisoner is being transported under the supervision of Interpol. Agent Mortimer Reyes, an ex-CIA agent from Texas, is a soon to be retired Interpol agent, with escorting the prisoner as his last assignment. Reyes along with agent Mina Milius & a small group escort travel through a route that leads them through the city of Pančevo, where they encounter an ecological holocaust and the people infected by the biohazard, who are trying to kill them. The two main agents soon realize that their only chance for escape from the zombie hordes lies in allying with the dangerous, mysterious prisoner.
