Il est la suite du film El Zorro escarlata en la venganza del ahorcado de Rafael Baledón. Luis Aguilar y reprend son rôle du Zorro escarlata.
Le film est divisé en trois épisodes : « El Doctor infernal » (Le Docteur infernal), « El esqueleto viviente » (Le Squelette vivant) et le titre du troisième est inconnu.
Synopsis
Le Docteur Kracken, un scientifique fou, cherche le secret de la vie éternelle. Il découvre un squelette animé parlant qui s'avère être la sorcière déjà aperçue dans le film précédent. Kracken l'aide à ranimer son monstrueux fils et cherche un moyen pour lui rendre son corps. Pour leurs expériences, ils envoient le monstre enlever de jeunes femmes.Actors

Alice (Milla Jovovich) and the others on the Umbrella Corporation freighter Arcadia face an attack by a fleet of tiltrotors led by Alice's former ally, Jill Valentine (Sienna Guillory), who is being mind controlled by Umbrella through a scarab device attached to her chest. The film opens with three gangsters cornering Santo (Rodolfo Guzmán Huerta) in a deserted alley and knock him out. He is taken to the laboratory of a crazed Doctor Campos (Joaquín Cordero) and turned into a docile servant of the mad doctor through a series of injections and electric shocks. Campos is apparently a respected scientist, the friend of Lieutenant Zambrano. Zambrano warns him to beware: other scientists have recently been kidnapped. Almost as they speak, the gangsters and the brainwashed Santo assault the bodyguards of a Doctor Lowel and carry off the scientist. He is later given the brainwashing treatment at Campos' lab. The bodyguards tell Zambrano that Santo, an undercover agent for the police force, was one of the kidnapers. Meanwhile, another undercover agent, El Incógnito (Fernando Osés) uses a special device to locate the hideout of Doctor Campos.

Quinn McKenna, membre d'un commando de Rangers américains, est témoin du crash d'un vaisseau spatial lors d'une mission de sauvetage d’otages. Il est le seul survivant. De son côté, Quinn a été arrêté par de mystérieux mercenaires, aux ordres de Will Traeger pour le Projet Stargazer. Ce dernier fait par ailleurs appel au docteure Casey Bracket, une exo-biologiste. Elle est accueillie dans un immense complexe dans lequel est conservé un Predator. Quant à Quinn, il se retrouve dans un bus avec d'anciens soldats ayant commis de graves délits et dont la plupart souffrent de troubles psychologiques : "Nebraska" Williams, Baxley, Coyle, Lynch et Nettles.

Telly Paretta (Julianne Moore) believes that her son, Sam (Christopher Kovaleski), died fourteen months ago in a plane crash, but her husband Jim (Anthony Edwards) tells her that she's delusional and that they have never had a son, and Eliot (Jessica Hecht) doesn't appear to believe in Sam's existence despite her closeness to him. Dr. Munce (Gary Sinise) tells her that Sam was merely a figment of her imagination and is just imagining a life that might have been. He recommends that she be sent to a hospital, but she runs away and meets with a man named Ash (Dominic West) who she thinks is the father of a girl (Kathryn Faughnan) who was friends with her son and died in the same crash. At first he dismisses her, claiming he never had a daughter, and calls the police. After she is taken into custody, however, he remembers his daughter and rescues Telly. Together they escape and go into hiding, pursued by National Security agents.
