The film opens with Peppino as a small child singing the popular song “Nel blu, dipinto di blu” with his brother in the back seat of a car on the way to a family gathering. The family is one of good standing in the social community and they are celebrating the fact that they have such a good life. In this scene the relationship between Peppino and his uncle Cesare is established. His uncle is a Don or Mafia boss in the small town of Cinisi where the story is set. In a scene soon after the happy family gathering we see don Cesare blown up by a car bomb which was planted by a rival Mafia boss. This ends Peppino’s time of innocence. After defeating Owen Shaw and his crew and securing amnesty for their past crimes, Dominic "Dom" Toretto, Brian O'Conner, and the rest of their team have returned to the United States to live normal lives again. In 1973, Sam "Ace" Rothstein (Robert De Niro) is a sports handicapper and Mafia associate who is sent to Las Vegas to run the Teamsters Union-funded Tangiers Casino on behalf of the Chicago Outfit. He hires old friend Billy Sherbert (Don Rickles) as his manager. In between, Ace and his friend, mob enforcer and caporegime Nicholas "Nicky" Santoro (Joe Pesci), narrate how the mob bosses control the Teamsters Union, which gives out money for casinos that they own, such as The Tangiers, and how they also drive off rival crews and get rid of cheaters. Ace becomes the Tangiers' de facto boss by taking advantage of lax gaming laws allowing him to work at the casino while his gaming license is still pending. He doubles the casino's profits, which are skimmed by the Mob before the records are reported to income tax agencies. The bosses are impressed with Ace's work and send Nicky to protect Ace and the whole business, along with Nicky's brother Dominick, Nicky's friend and subordinate Frank Marino (Frank Vincent), and the rest of Nicky's soldiers in his crew. Nicky, however, becomes more of a liability than an asset; his criminal activities- which he makes nearly no effort to conceal- and his violent and vicious temper quickly gets him banned by the gaming board from every casino, and his name is placed in the Black Book. Luca Di Angelo (Fabio Testi) is a smuggler, one member of an organized team trafficking cigarettes and booze up and down the coast off Naples, Italy. After a run-in with the police in which the smugglers manage to get away by faking a boat explosion resulting in the police motorboats responding to the false emergency allowing the smugglers to get away, Luca and his brother Mickey suspect Scherino (Ferdinand Murolo), the head of a rival gang of smugglers, of passing on their actives. Lucia and Mickey take their accusations to their boss Perlante (Saverio Marconi) a sleazy playboy withy numerous Mafia connections, who agrees to look into it. After a nighttime fire at Mickey's racing stables kills a valued racehorse, he and Luca drive over to inspect the damage. But on the way, they are stopped at a fake police roadblock where the assassins dressed as policemen trick Mickey into getting out of the car and machine-gun him to death over and over again (a homage to Sonny Corelone's death scene in The Godfather), while Luca barely escapes injury by hiding on the floor of the car.
