Like the first film, this follows the fraternal bond of two inmates, once again Ching, but this time with "Brother Dragon", a boss of the "Black Rats" gang. Ching escapes the prison in order to see his son, Leung was placed in an orphanage after his grandmother died, and gets into trouble with "Assassin"'s replacement, Officer Zau. Ching escapes a second time to follow Boss Dragon after he lied about jumping off a cliff to escape. Ching looked for his son and intended to bring him along, but was refused by the social worker.

Police officer Pitt's (Wan Yeung-ming) partner Lau was killed by the triads. The case also involves the seizure of a number of pornographic photos of politicians from an earlier investigation. The murder was witnessed by chef Fat Goose (Sammo Hung), and Pitt forces him to be the eyewitness and have the killer arrested. The killer, Lam Man-fu (Tommy Wong), however, was bailed out by his lawyer Kam Tse-ping (Lam Chung) and Pitt's superior Keung (Lau Kong) also forbids him from interfering with this case. Kyle LeBlanc (Van Damme) is an American working overseas in Russia. When he hears his wife being attacked over the phone, Kyle rushes home to find that he is too late. The man who has killed his wife is found not guilty on lack of evidence (but in fact because the defense bought the judge). Kyle, therefore, takes the law into his own hands and kills the man to avenge his wife's death, and for this he is sentenced to life in prison without parole. He soon befriends inmate Billy Cooper, a 21-year-old who is subjected to constant rape and beatings by prison inmates (they are actually helped by the guards who lock him in cells with these prisoners during the night) and also Malakai, a wheelchair-bound prisoner who claims to know the ins and outs of prison life.

The first part of the movie takes place in Bangkok, Thailand. Gou Fei's (Chow Yun-fat) friend Sam Sei (Anthony Wong) borrows money from a loan shark to give Gou Fei's departed mother a proper burial. The loan shark kidnaps Sam and Gao Fei rescues him by confronting the loan shark and persuade him to give more time for Sam to re-pay the loan. The loan shark does not comply and orders his cohorts to kill Sam but Gou Fei punches two of them before he engages a knife fight with the remaining gang members. After doing so, the loan shark attempts to shoot Gou Fei but he wrestles the gun out of the shark's grasp, frees a trapped Sam Sei and escapes. Not wanting to lose face, the loan shark promises to kill them, so Gou Fei and Sam Sei flee the city.

The plot mostly involves the two main characters, Ching and Yiu and problems they face with Mickey, a fellow inmate and "Scarface", a high-ranking officer. Yiu is sentenced to three years for manslaughter. In a fight for his father, he kicked a man onto the path of a bus. Ching was sent to prison for killing his wife for being a prostitute. During the end of the film, inmates decide to go on hunger strike to protest against the cigarette price rise, this causes problems with "Assassin". Ching bit "Assassin"'s ear off and gets an extended sentence, Yiu is released from the prison.

Crazy Lik and his friend Pong are two undercover cops who are sent to infiltrate triad leader Coffin Sing. Later as Sing is killed, the gang is split into two. Lik follows Hung Tai's side and Pong goes to the other. Lik becomes down as he is separated from his best friend and feeling responsible for Sing's death. This leads to a time where Lik gets drunk at his girlfriend Moon's mother's birthday party and making a fool of himself and Moon feels disgusted and leaves him. One time in a bar, Lik meets triad member Milky Fai, who helps Lik out to get good with Hung Tai and his sister Po. Milky Fai is actually a former undercover cop who has his own problems. Later on, Lik must decide whether or not to betray his triad brothers that he have grown very close to complete the case for his superior whom he really hates and is hitting on his girlfriend.
