The script was written principally by longtime Herzog production manager Walter Saxer, based on an idea from mountaineer Reinhold Messner, who Herzog had worked with in his documentary The Dark Glow of the Mountains. Herzog, who usually writes his own screenplays, believes that the script was weak, especially the dialogue, and says that he does not consider Scream of Stone to be his film.
The movie has elements drawn from the history of the supposed first conquest of the summit of Cerro Torre in 1959, by the Italian climber Cesare Maestri and his partner, the Austrian Toni Egger, who died during the descent.

August 2005, New Orleans, Louisiana. Terence McDonagh (Nicolas Cage) is a New Orleans Police Sergeant. While cleaning out a locker after Hurricane Katrina, he notices that a prisoner may not have been transferred. When he finds the prisoner about to drown, he mocks him before eventually jumping in the water to save him. He is promoted to lieutenant and given a medal for his work, but has suffered a serious back injury because of the rescue. As a result, he is prescribed Vicodin which he will most likely need to take for the rest of his life to manage the pain.

Based on the true story of the Detroit Red Wings' Ted Lindsay, a nine-time NHL All-Star who, along with Doug Harvey of the Montreal Canadiens, headed up a small group of players in a battle to protect the rights of players against monopolistic NHL owners of the 1950s era, including Bruce Norris of the Red Wings, Conn Smythe of the Toronto Maple Leafs and James D. Norris of the Chicago Black Hawks as well as NHL president, Clarence Campbell. The film focuses on the conflict between Lindsay and Jack Adams, Detroit's general manager, as well as Lindsay's struggle to win over the trust and support of the players, including Lindsay's long-time team mate, the legendary Gordie Howe, amidst coercion and threats from the league and the owners. Set in 1951 in South Korea, the 4077th Mobile Army Surgical Hospital is assigned two replacements: Captains "Hawkeye" Pierce and "Duke" Forrest, who arrive in a stolen Jeep. Both climbers successfully reach the summit of the previously unclimbed West Face of Siula Grande in Peru. Joe is injured during the descent after falling, resulting in a badly broken leg. The pair decide to lower Joe with the help of ropes on the steep snowy slope, while an enormous storm rages on. Simon cannot see where he is lowering Joe and Joe eventually drops off a cliff and is suspended by the rope in mid-air. Simon arrests his fall, but neither sees the predicament his partner is in nor hears him, due to the severity of the storm.

Dieter Dengler, a German-born U.S. Navy pilot in squadron VA-145, is shot down in his A-1 Skyraider over Laos in February 1966, while on a combat mission. He survives the crash only to be pursued, and ultimately captured, by the Pathet Lao. Dengler is given the chance for leniency by the Province Governor if he signs a document condemning America, but he refuses. He is tortured and taken to a prison camp. There he meets fellow American military soldiers and pilots, such as Gene DeBruin and Duane W. Martin, along with Y.C., Procet, and Phisit, some of whom have been captive for years. Dengler begins planning an escape, much to the disbelief of his fellow combatants, who have been downtrodden through physical and psychological torture by the camp guards.
