Jon Lansdale is finishing up a famous work of his called "Mandro", a Prince Valiant/Conan The Barbarian type of character. (In the film the actual "Mandro" artwork was drawn by real life Marvel Comics Conan artist Barry Windsor-Smith.) Lansdale is married to a beautiful woman named Anne (Andrea Marcovicci) and they have a child named Lizzie (played by Mara Hobel). While they are driving, Anne and Lansdale end up behind a slow-moving truck and an impatient driver behind them. In the heat of argument, Anne, who is driving, gets distracted and pulls out from behind the truck. Another truck comes up in the other lane. Anne attempts to get back behind the other truck but the impatient driver has pulled up too far. Lansdale waves his hand to the impatient driver to move back, and when she does not and Anne has to slam on the brakes, his hand is caught behind the truck, which also stops. The hand is severed completely in a bloody, violent scene. Anne attempts to find the severed hand but it is too late.

The story follows Casey (Nathan Forrest Winters), a normal boy whose life is constantly influenced by his intense fear of clowns. A disoriented man climbs up into the attic of a sorority house while the occupants hold a Christmas party. Jess Bradford (Olivia Hussey) receives an obscene phone call from "the moaner", a man who has recently been calling the house. After she calls sorority friends Barb Coard (Margot Kidder), Phyllis Phyl Carlson (Andrea Martin), Clare Harrison (Lynne Griffin) and the several other girls to listen, he starts saying perverted things to them, until Barb provokes the caller, to which he replies, "I'M GOING TO KILL YOU." Barb and Clare argue about the things she said to him, and Clare leaves. She is then attacked and killed; her body is carried up into the attic, where the killer places her in a rocking chair and puts a doll in her lap.

On October 31, 1978, picking up immediately after the events of the first film, Laurie Strode (Jamie Lee Curtis) is rushed to the hospital after being attacked by Michael Myers (Dick Warlock), who disappeared after being shot six times and falling from a second story balcony. Laurie begins having flashbacks of being adopted by the Strodes and visiting a boy in a mental institution. However, her sedation leaves her in a semi-conscious state most of the time. Paramedic Jimmy (Lance Guest) begins to develop romantic feelings for Laurie, despite the chagrin of the head nurse Mrs. Alves (Gloria Gifford), who is trying to get Laurie to rest comfortably. Dr. Loomis and Sheriff Brackett (Charles Cyphers) continue to search for Michael, only for an oncoming speeding police car to accidentally hit and kill Bennett Tramer (who was wearing a mask similar to Michael's) in the process.

In Hong Kong, Cheng Lai-sheung (Josie Ho) works two jobs with the hope of earning enough money to buy her own apartment with a view of the Victoria Harbour. Later in life, she vows to buy her mother and father a new apartment, but is unable to fulfill her promise before her mother dies. When her father becomes ill, she begins searching in earnest for a new place, having an obsession for the Victoria Bay No. 1 high-rise address from a childhood vow that she would one day buy a flat near the harbour so that her uncle would not have to walk there and back on a daily basis and not needing to search for him every evening.
