Synopsis
A patient escapes from a mental hospital, killing one of his keepers and then stealing his uniform. Three years later, a teacher is working late and gets stabbed and killed by the same patient, after he makes his way to the local college. Next semester, the late prof's replacement and a new group of students have to deal with a new batch of killings.Comments

A group of college friends--Harvey, Nikki, Rob, Skip, Nan, Chaz, Kit and Arch--gather to celebrate Spring Break by spending the weekend at the island mansion of their friend Muffy St. John, on the weekend leading up to April Fools' Day. The tone is set almost immediately, with Muffy preparing details around the house, and finds an old jack-in-the-box she remembers (in flashback). Adjie and Astrid, a married couple, are getting ready to travel to Sydney for a new job. They are accompanied by their friends, Alam, Eko, and Jimmy to the airport. At a gas station, Michelle Mancini (Natasha Gregson Wagner) fights off an apparent attack by a stuttering attendant (Brad Dourif). However, he was actually trying to warn her of an attacker in the back seat, and as Michelle drives off, the attacker in the back seat decapitates her with an axe. In 1978, Mary Mattock has her first period, and due to menstrual psychosis, murders her parents with a hatchet. Deemed mentally unfit, Mary is placed in Kings Park Psychiatric Center, where she is raped and impregnated by a guard in 1989. This incident is covered up, and after being told her child was a stillbirth, Mary goes on a rampage through the hospital. The police find Mary wandering the grounds, and open fire on her after she throws the severed head of her rapist at them, ending her killing spree. "Mary Hatchet" subsequently becomes a folklore figure, with local youngsters celebrating the Mischief Night-esque "Blood Night" in her name.
