Peter Neal, an American writer of violent horror novels, is in Italy to promote his latest work, Tenebrae. He is accompanied by his literary agent, Bullmer and his assistant, Anne. Neal is unaware that he has also been followed to Rome by his embittered ex-wife, Jane. Just before Neal arrived in Rome, Elsa, a young female shoplifter, was murdered with a razor by an unseen assailant. The murderer sends Neal a letter informing him that his books have inspired him to go on a killing spree. Neal is soon contacted by the police in the form of Detective Giermani and his female partner, Inspector Altieri.

A small California town is overrun with unruly and rowdy behavior from oil-field workers. Aaron Arnold, a Vietnam War veteran, and the brother of one of the locals (Ben Arnold), is hired to assist the police in restoring the peace. Aaron hires mercenaries trained in combat to help. After controlling the oil field workers, the veterans take over the town for their own not-always-legal purposes. Two bicyclists cycling on a canyon are followed by a mysterious black car down the road. At the bridge, the car proceeds to crush one cyclist against the wall, and ram the other from behind, causing him to fall off the bridge. A hitchhiker, hoping to get a ride, encounters the car and insults it after it purposefully tries to run him down. In response, the car runs over him and leaves. The local sheriff's office, called to the first of a series of hit and run deaths, get a lead on the car that appears heavily customized and has no license plate, as pointed out by Amos Clemens (R. G. Armstrong) after he sees it run over the hitchhiker.

Detective Ulisse Moretti (von Sydow) is investigating a series of murders in Turin in 1983 known as The Dwarf Murders. The main suspect, a giallo novelist named Vincenzo de Fabritiis, turns up dead and the case is considered closed. However, seventeen years later, a similar series of murders begin and draw the since-retired Moretti back into the case. Dr. Tim Mason (Roger Pryor), a medical researcher experimenting in "frozen therapy" visits the deserted home of Dr. Leon Kravaal (Boris Karloff), the originator of the therapy, who has been missing for ten years.
