Lady Torrance (Redgrave) is introduced as a disillusioned character, her Sicilian father was murdered by the Ku Klux Klan and she's been trapped in a miserable marriage for twenty years. West Germany, 1954. Lance Bombardier Evans, a sheltered middle-class National Serviceman, is about to be sent back to England to undertake a second attempt at officer training. But first he has to get through one night of guard duty without incident. Evans is in charge of a section of six men detailed to guard an obsolete Bofors Gun at a British military base. It soon becomes clear that none of the section, with the exception of Flynn, have any respect for Evans, guessing rightly that the latter has no enthusiasm and little ability in his role. Gunner O'Rourke in particular is troublesome and insubordinate, his contempt for Evans spurring him to test the authority and patience of the weak-willed NCO. Evan's fumbling attempts to engage him in friendly conversation only makes matters worse. The atmosphere grows more tense and O'Rourke strikes one of the other men, Rowe and then dares Evans to place him on a disciplinary charge but the NCO is too nervously intimidated to do so. O'Rourke and his sidekick Featherstone insist on being allowed to go to the NAAFI to buy cigarettes and Evans ill-advisedly lets them go.

The film depicts the relatively primitive medical practices of the time and the suppositions that physicians made in their efforts to understand the human body. After King George III begins to go mad, his doctors attempt cures such as blistering and purges, led on particularly by the Prince of Wales' personal physician, Dr. Warren. Meanwhile, another of the King's physicians, Dr. Pepys, analyses the King's stool and urine believing that body wastes may contain some clue to the Royal malady; of course, none of these attempts to cure the King actually works. Finally, Lady Pembroke, attendant to the Queen, recommends Dr. Willis, an ex-minister who attempts to cure the insane through behaviour modification, and who begins his restoration of the King's mental state by enforcing a strict regime of strapping the King into a waistcoat and restraining him whenever he misbehaves.

Handsome Cockney chauffeur Alfie Elkins (Michael Caine) enjoys the favors of women, while avoiding any commitment. He ends an affair with a married woman, Siddie (Millicent Martin), just as he gets his submissive single girlfriend, Gilda (Julia Foster), pregnant. Although Alfie refuses to marry Gilda and cheats on her constantly, Gilda decides to have the child, a boy named Malcolm, and keep him rather than give him up for adoption. Over time, Alfie becomes attached to his son, but his unwillingness to commit to Gilda causes her to break up with him and instead marry Humphrey (Graham Stark), a kindly bus conductor who loves her and is willing to accept Malcolm as his own son. She also bars Alfie from any further contact with Malcolm, forcing Alfie to watch from a distance as Humphrey steps into his fatherly role. When a health check reveals Alfie has tubercular shadows on his lungs, the diagnosis, combined with his separation from his son, leads him to have a brief mental breakdown.
