Four days after the events of the first film, detective James Carter is on vacation in Hong Kong, visiting his friend, Hong Kong Police Force Chief Inspector Lee, as he was sent along with Lee in saving the life of the Chinese counsel's daughter in Los Angeles. The fun is put on hold when a bomb explodes at the United States Consulate General, killing two undercover U.S. custom agents inside. The film starts with a framing device of psychologist Dr. Arnold (Patrick Bauchau) in a session with Karla Homolka (Laura Prepon) at Canada's Regional Psychiatric Centre in Saskatoon, Saskatchewan. Dr. Arnold's evaluation will determine Karla's eligibility for parole. During the session, Dr. Arnold shows Karla a photo album of herself and her husband Paul Bernardo (Misha Collins) in happier times, and Karla has a memory of how they first met. However, when Dr. Arnold introduces the subject of Karla's sister Tammy, Karla becomes uncooperative. Over the following weeks, Dr. Arnold probes Karla about her knowledge of Paul's secret life as a serial rapist, and it is revealed that shortly before their marriage Paul began to rape women.

The divorced cop Eli Cantor once brought a man called Simon Leenhardt behind bars. Leenhardt has sworn revenge to Cantor. For many centuries, the phi song neng (vampires who have vowed never to drink human blood) have lived among the villages of the Far East. One day over 800 years ago, a young human warlord named Niran fell in love with one of the song neng, a woman named Sang. Niran wanted Sang for his own, so he killed her husband and son. In her pain and rage, Sang attacked and bit Niran during a Songkran (New Year) solar eclipse, turning him into a vampire. From Niran's bloodline have come many jai tham (vampires who drink human blood and kill for pleasure). Only Sang can end the jai tham's nightly attacks on humans. Doing so, however, will end the entire bloodline and turn every vampire, song neng as well as jai tham, into mere mortals again. Since the coming Songkran festival will feature the first solar eclipse in 800 years, Sang intends to kill herself by exposing herself to the sun's rays at the end of the eclipse and thus wipe out all the vampires in Thailand.

At St. John's Orphanage in 1882, children, including a young Albert Fish, are being paddled as punishment for their sins. Albert Fish as an adult (Patrick Bauchau) then tells an anecdote of a horse that some older boys at the orphanage set on fire, comparing himself to the horse. He regularly whips himself with a belt while hallucinating himself as he appeared in the orphanage. Fish kills a boy scout, Francis McDonnell, before visiting the Budd family home, where he abducts and murders ten-year-old Grace Budd (Lexi Ainsworth) on June 3, 1928 under the pretense of taking her to his niece's birthday party. Throughout the film is a film noir-style narration by Detective William King (Jack Conley), of the Missing Persons Bureau. King searches for Grace Budd for six years, before finally tracking down and arresting Fish. Fish is found guilty despite evidence of his insanity, and promptly sentenced to die in the electric chair.
