It is a dramatic piece about a fifteen-year-old boy taking a road trip with his recently widowed grandmother. The piece is notable in that it was Field's first project as a writer/director and his first introduction to many of the collaborators he continued to work with on subsequent projects, including his Academy Award nominated features In the Bedroom and Little Children.

The film is set in the Mid-Coast town of Camden, Maine. Matt (Tom Wilkinson) and Ruth Fowler (Sissy Spacek) enjoy a happy marriage and a good relationship with their son Frank (Nick Stahl), a recent college graduate who has come home for the summer. Frank has fallen in love with an older woman with children, Natalie Strout (Marisa Tomei). Frank is also applying to graduate school for architecture, but is considering staying in town to work in the fishing industry and be near to Natalie. Natalie's ex-husband, Richard Strout (William Mapother), tries to find a way into his ex-wife and son's lives, going to increasingly violent lengths to get his intentions across to Natalie. Ruth is openly concerned about Frank's relationship with Natalie, while Matt thinks it is only a fling.

Sarah Pierce (Kate Winslet) is a hapless, stay-at-home mother in a small suburban Boston community. She had been working on a doctorate in English, but set aside her work to marry Richard (Gregg Edelman) and raise their 3-year-old daughter, Lucy (Sadie Goldstein). Her marriage falls apart when she discovers that Richard is addicted to online pornography. She meets Brad Adamson (Patrick Wilson), a law student who brings his 4-year-old son, Aaron (Ty Simpkins), to the park. Set in 1761 in London, the film focuses on events at St. Mary's of Bethlehem Asylum, a fictionalized version of Bethlem Royal Hospital, also known as "Bedlam." After an acquaintance of aristocrat Lord Mortimer dies in an attempt to escape from the asylum, apothecary general Master George Sims (played by Karloff, a fictionalized version of an infamous head physician at Bethlem, John Monro) appeases Mortimer by having his "loonies" put on a show for him. Mortified by the treatment of the patients, Mortimer's protégé Nell Bowen (Lee) seeks the help of Whig politician John Wilkes to reform the asylum. Mortimer and Sims conspire to commit Nell to the asylum, where her initial fears of the fellow inmates do not sway her sympathetic commitment to improving their conditions. Frustrated by Nell's progress with the inmates, Sims threatens her with his strongest "cure" but his attempt is thwarted by the very inmates that Nell helped. Ultimately, Sims is literally "deposed" and Nell is rescued by her Quaker friend who had counselled her through the whole process.

Le père de Jason Smith, sur son lit de mort, lui fait promettre de disperser ses cendres sur la tombe de sa femme, la mère de Jason, morte alors que Jason était encore très jeune. Alors qu'il est dans le cimetière pour accomplir sa promesse, il remarque avec étonnement que presque toutes les tombes indiquent une date de décès identique, le même jour, le même mois, mais avec une année d'écart à chaque fois. Face à cette étonnante constatation, il fouille dans sa mémoire et lui revient alors un souvenir flou d'une loterie particulière qui se tenait dans ce village…
