L'an 2563, 300 ans après que la Terre a connu une catastrophe appelée "l'effondrement" consécutive à une guerre contre Mars. In 2028, multinational conglomerate OmniCorp revolutionizes warfare with the introduction of robotic peacekeepers capable of maintaining law and order in hot spots such as Iran. In January 1865, President Abraham Lincoln expects the Civil War to end within a month. However, he is concerned that his 1863 Emancipation Proclamation may be discarded by the courts once the war has concluded and that the Thirteenth Amendment be defeated by the returning slave states. Lincoln feels it is imperative to pass the amendment by the end of the month, thus removing any possibility that slaves who have already been freed may be re-enslaved. The Radical Republicans fear the amendment will be defeated by some who merely wish to delay its passage; the support of the amendment by Republicans in the border states is not yet assured either, since they prioritize the issue of ending the war. Even if all of them are ultimately brought on board, the amendment will still require the support of several Democratic congressmen if it is to pass. With dozens of Democrats having just become lame ducks after losing their re-election campaigns in the fall of 1864, some of Lincoln's advisors believe that he should wait until the new Republican-heavy Congress is seated, presumably giving the amendment an easier road to passage. Lincoln, however, remains adamant about having the amendment in place and the issue of slavery settled before the war is concluded and the southern states readmitted into the Union.

In 1954, two U.S. Marshals - Edward "Teddy" Daniels and his new partner, Chuck Aule - travel to the Ashecliffe Hospital for the criminally insane on Shutter Island located in Boston Harbor as part of an investigation into the disappearance of patient Rachel Solando, incarcerated for drowning her three children. The sole clue is a note left by Solando, which reads: "The law of 4; who is 67?" Shortly after arrival, a storm prevents their return to the mainland for several days. Daniels finds the staff confrontational: the lead psychiatrist, Dr. John Cawley, refuses to hand over records of the hospital staff; Solando's doctor, Dr. Lester Sheehan, had left on vacation after her disappearance, and they are barred from searching Ward C and told that the lighthouse on the island has already been searched, so there is no need to search it. When a patient is being interrogated by Daniels, with a subterfuge she sends Aule away for a few seconds and scribbles "RUN" in Daniels' notepad.

Kris Fowles (Katie Cassidy) goes to the Springwood Diner to meet with her boyfriend, Dean Russell (Kellan Lutz), who falls asleep at the table and meets a man covered in burn scars, wearing a red and green sweater, a fedora and a clawed glove on his hand. The burned man cuts Dean's throat in the dream, but in reality it appears that Dean is cutting his own throat as his friend, waitress Nancy Holbrook (Rooney Mara), looks on with Kris. At Dean's funeral, Kris sees a photograph of her and Dean as children, but cannot recall ever knowing Dean before high school. Kris begins to dream about the burned man herself and refuses to go to sleep for fear that she will die in her dreams. Jesse Braun (Thomas Dekker), Kris's ex-boyfriend, shows up at her house to keep her company while she sleeps, but Kris meets the burned man in her dreams and is murdered. The "Minutemen", a team of costumed crime fighters, was formed in 1939 in response to a rise in costumed gangs and criminals; the "Watchmen" was similarly formed decades later. Their existence has dramatically affected world events: in particular, Doctor Manhattan's superpowers help the United States win the Vietnam War, resulting in President Richard Nixon being reelected. Doctor Manhattan gives the West a strategic advantage over the Soviet Union, which by the 1980s threatens to escalate the Cold War into a nuclear war. During that time, growing anti-vigilante sentiment in the country leads to masked crime-fighters being outlawed. While many of the heroes retire, Doctor Manhattan and the Comedian operate as government-sanctioned agents, and Rorschach continues to operate outside the law.

In 1976, Jackie Moon is a singer who has used the profits from his one hit single "Love Me Sexy" to buy a basketball team in the American Basketball Association, the Flint Tropics, becoming the owner, head Coach, and starting power forward. The ABA Commissioner announces a plan to merge the league with the National Basketball Association, but only four teams will move to the more established league. The Tropics, the worst team in the league, are in danger of dissolving. In response, Jackie argues that the teams with the four best records overall should be merged into the NBA.
