The film is about a prisoner awaiting execution. Denver Bayliss (Kiefer Sutherland) gains an unlikely friend and confidant in Fred Whitmore (Forest Whitaker), a former cop who is now a prison guard. The film opens in the countryside outside the fictional town of Lahood, California, and takes place sometime in the 1870s or early 1880s (based on remarks in the film about outlawing hydraulic mining). Thugs working for big-time miner Coy LaHood ride in and destroy the camp of a group of struggling miners and their families who have settled in nearby Carbon Canyon and are panning for gold there. In leaving, they also shoot the little dog of fourteen-year-old Megan Wheeler. As Megan buries it in the woods and prays for a miracle, we see a stranger heading to the town on horseback.

In 1943, the Nazi military occupies Rome. Pope Pius XII (John Gielgud) is approached by General Max Helm and SS Head of Police for Rome Lieutenant Colonel Herbert Kappler (Christopher Plummer). The Colonel expresses concern that escaped Allied prisoners may attempt to seek refuge in the Vatican, and requests permission to paint a white line across St. Peter's Square in order to mark the extent of Vatican sovereignty. A mute Scotswoman named Ada McGrath is sold by her father into marriage to a New Zealand frontiersman named Alisdair Stewart, bringing her young daughter Flora with her. The voice that the audience hears in the opening narration is "not her speaking voice, but her mind's voice". Ada has not spoken a word since she was six years old and no one, including herself, knows why. She expresses herself through her piano playing and through sign language, for which her daughter has served as the interpreter. Flora later dramatically tells two women in New Zealand that her mother has not spoken since the death of her husband who died as a result of being struck by lightning. Ada cares little for the mundane world, occupying herself for hours every day with the piano. Flora, it is later learned, is the product of a relationship with a teacher with whom Ada believed she could communicate through her mind, but who "became frightened and stopped listening," and thus left her.

Will Randall is bitten by a wolf while driving home in Vermont. Afterwards, he is demoted from editor-in-chief of a publishing house when it is taken over by tycoon Raymond Alden, who replaces him with Will's protégé Stewart Swinton. When Will thinks that his wife Charlotte is having an affair with Stewart, he rushes over to his house where Stewart sees Will angry and asks him what the problem is. Arthur Banks (Sutherland) is an Ivy League-educated hitman for Latin American drug cartels who picks up solitary women, uses them as cover for a hit, then kills them. His trademark is a bullet with a tungsten core. Over the years, he has become wary of the FBI's attempts to catch him, including by use of satellite and security cameras, which leads him to mostly stay in rural desert areas when not working. The FBI team is spearheaded by Agent George Scanlon (Sheridan), who lost five years of his career when Banks killed a witness he was guarding 15 years ago while leaving no evidence behind. In a desperation sting, Scanlon plants Agent Bennie Harper (Walters), portraying a drifter, in Banks' path, and Banks picks her up for what he says will be his last job, a hit on a Mexican presidential candidate. Scanlon and Agent Donna Marbury (Stefanson), along with several support agents, follow Banks and Harper through the Southwest, but the scheme goes wrong when one of the tailing agents is spotted and caught by Banks.

In Heaven (which resembles a modern police headquarters), angels are tasked with ensuring that mortals on Earth find love.
