Ross (Jason Schwartzman) is a customer of Spider Mike (John Leguizamo), a methamphetamine dealer. Spider Mike and his girlfriend Cookie (Mena Suvari) are constantly arguing, and Ross strikes up a friendship with Nikki (Brittany Murphy), a fellow addict. Nikki takes Ross to meet her boyfriend, "The Cook" (Mickey Rourke), who supplies Spider Mike with drugs from a meth lab he has set up in a motel room. The film opens in early 1980s Canada where teenager Martin Asher (Paul Dano) is seen ambling about a bus station, seemingly uncertain of his destination. He befriends another teen on the bus, Matt Soulsby (Justin Chatwin) and the two talk about their plans for the future. When their bus breaks down, the two acquire a car from a nearby garage. While Martin is driving, a tire blows. Matt struggles to change the tire and Martin comments on how he and Matt are both about the same height, and kicks Matt into the path of an oncoming truck. He is last seen toting Matt's guitar and walking away singing in a voice similar to Matt's.

On her piano, eight year old Rhoda Penmark (Patty McCormack) plays the French song "Au clair de la lune", while her father (William Hopper) says his goodbyes to her and his wife, Christine (Nancy Kelly), as he goes away on military duty. Their neighbor and landlord, Monica Breedlove (Evelyn Varden), comes in with a present for Rhoda - a locket. Rhoda, looking pristine and proper in her perfect pinafore dress and long blonde pigtails, thanks Monica for the gift. She then tap dances on the hard floor. Monica notices Rhoda's tap shoes, and Rhoda says that adding the taps to the shoes was her own idea. They then discuss a penmanship medal competition that Rhoda lost to her schoolmate, Claude Daigle; Monica speaks of it as a childish disappointment, but Rhoda's face darkens with fury. Christine and Rhoda leave for the school picnic at a nearby lake.

As Francis (Friedrich Feher) sits on a bench with an older man who complains that spirits have driven him away from his family and home, a dazed woman named Jane (Lil Dagover) passes them. Francis explains she is his "fiancée" and that they have suffered a great ordeal. Most of the rest of the film is a flashback of Francis' story, which takes place in Holstenwall, a shadowy village of twisted buildings and spiraling streets. Francis and his friend Alan (Hans Heinrich von Twardowski), who are good-naturedly competing for Jane's affections, plan to visit the town fair. Meanwhile, a mysterious man named Dr. Caligari (Werner Krauss) seeks a permit from the rude town clerk to present a spectacle at the fair, which features a somnambulist named Cesare (Conrad Veidt). The clerk mocks and berates Dr. Caligari, but ultimately approves the permit. That night, the clerk is found stabbed to death in his bed.

On October 6, 1970, while on holiday in Istanbul, Turkey, American college student Billy Hayes straps 2 kg of hashish blocks to his chest. While attempting to board a plane back to the United States with his girlfriend, Billy is arrested by Turkish police on high alert due to fear of terrorist attacks. He is strip-searched, photographed and questioned. After a while, a shadowy American (who is never named, but is nicknamed "Tex" by Billy due to his thick Texan accent) arrives, takes Billy to a police station and translates for Billy for one of the detectives. On questioning Billy tells them that he bought the hashish from a taxicab driver, and offers to help the police track him down in exchange for his release. Billy goes with the police to a nearby market and points out the cab driver, but when they go to arrest the cabbie, it becomes apparent that the police have no intention of keeping their end of the deal with Billy. He sees an opportunity and makes a run for it, only to get cornered and recaptured by the mysterious American.
