The film opens with homemade videos of fraternal twins Jack and Jill Sadelstein growing up. As the videos progress, it seems that Jack is the more gifted twin, with Jill constantly trying to get his attention by hitting him, hurting girls around him, etc. The story shifts to the present, where an adult Jack (Adam Sandler) is a successful advertising executive in Los Angeles with a beautiful wife, Erin (Katie Holmes), and two kids, Sofie (Elodie Tougne) and Gary (Rohan Chand), while Jill (Sandler) never left the working-class neighborhood they grew up in. At work, Clark Griswold (Chevy Chase) has invented a long life food preservative, earning him a large bonus check. Clark announces to his family that he is taking them on vacation. Enthusiasm wanes, however, when Clark says they are headed to Las Vegas, Nevada. In the beginning chapter, "Serra Pelada", men from Serra Pelada (a gold mine in Brazil) are seen carrying bags of dirt up to a destination. In the middle of the chapter, various shots outside of Serra Pelada are shown. Near the end of the chapter, a few men are carrying another man who was struck by a falling rock (mentioned in the "Impact of progress" feature on the DVD) uphill along a procession of workers who are carrying dirt filled sacks. After that, several discordant layered exposures of the dirt carriers are shown. The scene cross fades to show the image of a head, with multiple exposures of the same head rapidly rotating and layered upon to give a manifold appearance. This is an apparent allusion to Janus, the god of beginnings, endings and transitions, keeping with the film's central themes of progress and change. After that, the film's title is shown in red.

A brilliant young trainee can't stand the sight of blood. A doctor romances the head nurse in order to get the key to the drugs cabinet. There's a mafioso on the loose disguised as a woman - in other words all the usual ingredients present and correct, though in this case the laughs are intentional.
