In the year 2008, heavy rainfall has flooded large areas of London. Rookie police officer Dick Durkin (Duncan) is assigned to partner Harley Stone (Hauer), a burnt-out and highly cynical homicide detective who, according to his commanding officer, survives on "anxiety, coffee and chocolate" after being unable to prevent the murder of his partner by a serial killer several years previously. Now however, the murders have begun again and Stone and Durkin are assigned the case. After investigating the scenes of several killings, they appear no closer to identifying the killer, with their only clues being that the murders seem to be linked to the lunar cycle, and that the killer has multiple recombinant DNA strands, having absorbed the DNA of the victims.

He travels to East Germany to perform at a cultural festival, and at a dinner, he sees the beautiful Hillary Flammond (Gutteridge), a member of the resistance movement, attempting to avoid the authorities. He pretends to be her date to get to know her, and performs an impromptu song and dance ("Tutti-Frutti") mistakenly thinking that he was asked to do so, to the delight of Hillary and the crowd but to the annoyance of the emcee who intended to introduce someone else. He later sees Hillary at a ballet, where she is trying to meet with a resistance contact, but is met by the police instead. Nick saves her and they try to escape, but Nick turns himself in so that Hillary can get away. He is taken to a prison where he is questioned and tortured, but he knows nothing and does not break. In an escape attempt, he ends up in the secret lab of Dr. Paul Flammond (Gough), a brilliant scientist developing the "Polaris naval mine", a device that can destroy the entire NATO submarine fleet; the Germans force him to work by threatening to kill his daughter Hillary. When attempts to break into the pop business leaves him with nothing but a bloody nose, songwriter Jason Jones (Robin Askwith) decides to take a break with "Hairy Holidays", an outfit run by shifty, gay travel agent Pollock (Dennis Price). After failing to chat Jason up, Pollock sends him to pseudo-health farm Brittlehurst Manor. On the train journey there Jason meets Judy (Vanessa Shaw) who is also on the way to the same destination to meet her long lost aunt. Both are unaware that the health farm (i.e. "Horror Hospital") is a front for Dr. Storm (Michael Gough) and his lobotomy experiments that turn wayward hippies into his mindless zombie slaves. The wheelchair-bound doctor surrounds himself with an entourage that includes Judy's aunt, erstwhile brothel madam Olga (Ellen Pollock), dwarf Frederick (Skip Martin) and numerous zombie biker thugs. Dr. Storm also has a Rolls Royce fitted with a giant blade that decapitates escapees and interfering parties. Abraham (Kurt Christian) arrives at the Horror Hospital "looking for his chick" and is promptly whacked around the head by the motorcycle zombies. Frederick, fed up at literally being Storm's whipping boy helps the kids escape—paving the way for the 70s youth to put the final spanner in the works to Storm's scheme.

Decker, who convinces Boone that he committed a series of murders. In reality, Decker is a masked serial killer who has murdered several families. Decker drugs Boone with LSD disguised as lithium and orders Boone to turn himself in. Before he can, Boone is struck by a truck and taken to a hospital. There, Boone overhears the rants of Narcisse who seeks to enter Midian. Narcisse, convinced that Boone is there to test him, gives Boone directions before tearing the skin off his face. Boone makes his way to Midian, a city standing under a massive graveyard. Once there, he encounters supernatural creatures Kinski and Peloquin. Kinski says that they should bring him below, but Peloquin refuses to allow in a normal human. Boone claims to be a murderer, but Peloquin smells his innocence and attacks him. Boone escapes, only to run into a squad of police officers led by Decker. Boone is gunned down by the police after Decker tries to get him to turn himself in. Due to Peloquin's bite, he comes back to life in the morgue. When he returns to Midian, he finds Narcisse there and he is inducted into their society by Dirk Lylesburg, leader of the Nightbreed. In an initiation ceremony, he is touched by the blood of their deity Baphomet.

The film opens with a mysterious man, later referred to as "Byron" (portrayed by Bob Peck), running down a canyon, being harassed by an aeroplane. Byron takes refuge from his pursuers on a precipice overlooking the canyon. The plane lands and its occupants, two obsessive law enforcement officers named Will Tasker and Belitski (Mark Hamill and Kitty Aldridge), chase the man on foot. They shoot the fugitive through the arm with a grappling hook. The fugitive looks at his arm, but does not seem injured, only intrigued. Tasker pulls on the rope now connected to the fugitive. He tumbles down the side of the canyon, but once again is not harmed. Immediately after his fall, the fugitive recites the words of famous World War II aviator and poet John Gillespie Magee, Jr., "I have slipped the surly bonds of Earth - put out my hand and touched the Face of God.", from the poem High Flight.
