In AD 180, Spanish-Roman General Maximus Decimus Meridius leads the Roman army to a decisive victory against the Germanic tribes near Vindobona, ending a long war on the Roman frontier and winning the favor of the Emperor Marcus Aurelius. Maximus, weary of battle, decides to retire at his Spanish farm estate at the Emperor's behest, but the Emperor tells him that his son and heir to the throne, Commodus, is unfit to rule and thus appoints him as regent to help save Rome from corruption. Before he makes his decision, Commodus is confronted by his father who tells him that he will not be emperor. Burdened by grief and believing he was never valued by his father, Commodus kills him.

The film opens with newsreel footage, including the farewell address in 1961 of outgoing President Dwight D. Eisenhower, warning about the build-up of the "military-industrial complex". This is followed by a summary of John F. Kennedy's years as president, emphasizing the events that, in Stone's thesis, would lead to his assassination. This builds to a reconstruction of the assassination on November 22, 1963. New Orleans District Attorney Jim Garrison subsequently learns about potential links to the assassination in New Orleans. Garrison and his team investigate several possible conspirators, including private pilot David Ferrie, but are forced to let them go after their investigation is publicly rebuked by the federal government. Kennedy's suspected assassin Lee Harvey Oswald is killed by Jack Ruby, and Garrison closes the investigation.

A deadly virus wipes out almost all of humanity in 1996, forcing remaining survivors to live underground. Nick Rivers (Kilmer), a United States rock star, has the number one song in America ("Skeet Surfing"). He travels to East Germany to perform at a cultural festival, and at a dinner, he sees the beautiful Hillary Flammond (Gutteridge), a member of the resistance movement, attempting to avoid the authorities. He pretends to be her date to get to know her, and performs an impromptu song and dance ("Tutti-Frutti") mistakenly thinking that he was asked to do so, to the delight of Hillary and the crowd but to the annoyance of the emcee who intended to introduce someone else. He later sees Hillary at a ballet, where she is trying to meet with a resistance contact, but is met by the police instead. Nick saves her and they try to escape, but Nick turns himself in so that Hillary can get away. He is taken to a prison where he is questioned and tortured, but he knows nothing and does not break. In an escape attempt, he ends up in the secret lab of Dr. Paul Flammond (Gough), a brilliant scientist developing the "Polaris naval mine", a device that can destroy the entire NATO submarine fleet; the Germans force him to work by threatening to kill his daughter Hillary. Nick is recaptured and scheduled for execution.
