In 1929, Italian dictator Benito Mussolini (Rod Steiger) is still faced with the 20-year-long war waged by patriots in the Italian colony of Libya to combat Italian colonization and the establishment of "The Fourth Shore"—the rebirth of a Roman Empire in Africa. Mussolini appoints General Rodolfo Graziani (Oliver Reed) as his sixth governor to Libya, confident that the eminently accredited soldier and fascist Grande can crush the rebellion and restore the dissipated glories of Imperial Rome. Omar Mukhtar (Anthony Quinn) leads the resistance to the fascists. A teacher by profession, guerrilla by obligation, Mukhtar had committed himself to a war that cannot be won in his own lifetime. Graziani controls Libya with the might of the Italian Army. Tanks and aircraft are used in the desert for the first time. The Italians also committed atrocities: killing of prisoners of war, destruction of crops, and imprisoning populations in concentration camps behind barbed wire.

The film begins with a woman named Sheila who is searching for her sister, who has disappeared in the southeastern jungles of Asia (India). Sheila (Janet Agren) joins up with Mark (Robert Kerman), and they both encounter many perils while searching for Sheila's sister, Diana (Paola Senatore). Diana has joined a cult run by a man called Jonas (Ivan Rassimov). Jonas physically and sexually abuses his followers and local people alike. In one graphic scene, he rapes Sheila with a dildo covered in snake blood, and decapitates a native. In another, a native widow named Mowara (Me Me Lai) is ritualistically raped after her late husband's body is burned on a pyre. A group, consisting of Mowara, Mark, Sheila and Diana escapes into the jungle, where Diana and Mowara are caught by a group of cannibals, Diana raped, and then both hacked to death, while Mark and Sheila helplessly watch from the cover of the bushes. They quickly escape back to New York when helicopters sent by the authorities come looking for them. Back in the village, the rest of the cult commits ritual suicide, leaving one young female survivor for the authorities to find.

Investigating an abandoned boat in a New York harbour, a patrolman is killed by a zombie before his partner chases it overboard; the dead patrolman's body is taken to the morgue. Anne Bowles (Tisa Farrow) is questioned by police, as the boat belonged to her father. She claims he is conducting research on Matul, a Caribbean island. A newspaper reporter, Peter West (Ian McCulloch), is investigating the story; he and Bowles learn that Bowles' father is suffering from a strange illness on the island. After the American Civil War gunman California wants to renew his life. He changes his name to Michael Random and seeks a home where he can live in peace.
