Melvin Smiley (Mark Wahlberg) is a hitman leading a secret life and maintaining two relationships, one with the demanding and demeaning Chantel (Lela Rochon), who does not accept his work, and another with Pam (Christina Applegate), who knows nothing of his job. Melvin is somewhat of a pushover, trying to appease all of Chantel's demands, even her most expensive wishes, as well as rolling over whenever one of his co-workers takes credit for his achievements. Perhaps as a result of his helplessness in asserting himself, throughout the early scenes Melvin is often seen drinking Maalox to relieve an incipient ulcer.

James and Mollie, after struggling to put Mikey (now 7 years-old) and Julie (now 4 years-old) to bed, again have sex in their bedroom. She tries to refuse but he charms his way through her resistant behavior. Meanwhile outside, a pair of different breed dogs, having come from a night out are about to part ways. But the male, Trooper, instead cons the female, Princess, into mating with him after he lies and says that he is going to be fixed the next day. The screen changes for the credits of several of Trooper's sperm cells entering Princess' eggs. Their owners discover that she has given birth and decide simply to put them up for adoption.

Dr. Jeff Weitzman (Dennis Boutsikaris) is a psychologist working in a sanitarium in New Jersey. His primary patients are Billy, Henry, Jack and Albert. Billy (Keaton) is the most normal of the group and their unofficial leader, though he is a pathological liar with delusions of grandeur and violent tendencies. Henry (Lloyd) is obsessive/compulsive and he has deluded himself into thinking he is one of the doctors at the hospital, often walking around with a clipboard, lab coat and stethoscope. At a school called P.S. 5, a teacher named Miss Walker tells some children a version of E. T. A. Hoffmann's The Nutcracker and the Mouse King, involving the Care Bear Family. In Care-a-lot, the Care Bears are visited by the White Rabbit, the uncle of Swift Heart Rabbit. The White Rabbit gives the Care Bears the task of finding the missing Princess of Heart, who is to be crowned queen in Wonderland, otherwise the villainous Wizard of Wonderland will gain the throne. Tenderheart, Grumpy, Good Luck, Brave Heart, Lotsa Heart, Swift Heart and the White Rabbit search all over the world for the Princess, but to no avail. Grumpy is pointed to a girl who resembles the Princess, Alice. The Care Bears decide that Alice could act as the Princess until the real one is found. The group is separated by the power of the Wizard, forcing Grumpy, Swift Heart and the White Rabbit to use a rabbit hole to reach Wonderland.
