Unable to leave the germ-free confines of his sterilized bedroom for any real stretch of time, environmentally hypersensitive Marvin (Karl Geary) spends his days taking pictures of his neighbors from his window. Occasionally catching glimpses of his young sister Rosy (Alexis Dziena) hanging out with the neighborhood drug dealer, Marvin's lens remains mostly fixed on a mysterious neighbor known as the Garbageman (Lance Henriksen) and pretty neighbor Carmen (Rebecca Mader), while his slightly overbearing mother (Amanda Plummer) rests on the couch. As neighbors begin disappearing and mysterious figures move in and out of Marvin's viewfinder, the secluded voyeur begins to suspect that a sinister force is at work in his neighborhood. Though Rosy and Carmen are anxious to assist in a little detective work, the situation soon begins to spiral out of control upon the discovery that the Judas Breed is far from extinct.

On the Valentine's Day of 1997, in a small town, a cave-in on the north side of a Hanninger mine trapped six miners. Six days later, rescue teams found five dead miners and the comatose Harry Warden (Richard John Walters), who survived by killing the other miners with a pickaxe, allowing himself to breathe. Tom Hanniger (Jensen Ackles), the mine owner's son, was blamed for the mine disaster because he forgot to vent the methane lines and caused a cave-in, but also Harry Warden for killing the miners.

Danyael Rosales is a street preacher who thinks God does not care about anyone because of the death of his parents, Valerie Rosales and the angel Danyael from the previous film. He is then forced to face his destiny. As a Nephilim, he has some of the angels' abilities, such as regeneration, and can only be killed if his heart is removed. One night, a blind assassin shoots Danyael as he preaches before a crowd, but the assassin is driven off before he can take out Danyael's heart. As punishment for his failure, Zophael kills the assassin and goes after Danyael himself with an extendable weapon with a blade that can be turned into a three-pronged hook. However, Danyael is protected by Gabriel, a now-human fallen angel who killed Danyael's father and performed many misdeeds. After being defeated by Danyael's mother, Gabriel was turned into a human as punishment. Having spent years as a human, he now realizes how wrong he was in the past.

Matthew Van Helsing (Christopher Plummer), the alleged descendant of the famed 19th century Dutch medical doctor, Abraham Van Helsing, owns an antique shop in London in 2000. Based on the level of security surrounding the coffin, Solina and Marcus decide that the coffin’s contents must be valuable, so they escape with it and flee to New Orleans, Louisiana. When Van Helsing discovers that the coffin has been stolen, he boards a plane to America, telling his apprentice, Simon Sheppard (Jonny Lee Miller), to remain in London. Simon does not follow his mentor’s orders and travels to Louisiana as well.
