In late 1890s Wyoming, Butch Cassidy (Paul Newman) is the affable, clever, talkative leader of the outlaw Hole in the Wall Gang. His closest companion is the laconic dead-shot "Sundance Kid" (Robert Redford). The two return to their hideout at Hole-in-the-Wall (Wyoming) to discover that the rest of the gang, irked at Butch's long absences, have selected Harvey Logan (Ted Cassidy) as their new leader. Harvey challenges Butch to a knife fight over the gang's leadership. Famed novelist Paul Sheldon (James Caan) is the author of a successful series of Regency romance novels featuring a character named Misery Chastain. Wanting to focus on more serious stories, he writes a manuscript for the final Misery novel. While traveling from Silver Creek, Colorado to his home in New York City, he is caught in a blizzard and his car goes off the road, rendering him unconscious. Paul is rescued by a nurse named Annie Wilkes (Kathy Bates), who brings him to her remote home. When Paul regains consciousness he finds himself bedridden, with both his legs broken as well as a dislocated shoulder. Annie claims she is his "number one fan" and talks a lot about him and his novels. As a reward for saving him, Paul gives Annie his new manuscript which she saved from the wreckage. While feeding him, she is angered and spills soup on him but regains control and apologizes. She buys a copy of Paul's most recently published book, Misery's Child, giving glowing praise to Paul as she progresses through the book. However, when Annie discovers that Misery dies at the end of the book she flies into a rage, almost smashing a table on Paul's head. She reveals that she lied about calling his agent and the authorities; nobody knows where he is. Annie leaves and Paul tries to escape from his room, but she has locked the door.

1930's France. A stranger on horseback rides into the isolated mining town of Lago. Three gun-toting men follow him into the saloon, taunting him to fight. When they follow him to the barbershop and begin threatening him, the Stranger quickly dispatches the three with little effort. Impressed with this performance, a dwarf named Mordecai, who works in the barbershop, befriends the Stranger. A woman named Callie Travers arranges to bump into the Stranger in the street, claiming loudly it was his fault. When she slaps the cigar from his mouth while insulting him, he drags her into the livery stable and has sexual relations with her in a very rough and aggressive manner (the scene is initially depicted as a rape, but by its end, the woman has clearly given her consent, as she is portrayed as an active, and seemingly enthusiastic participant in the act). Next, the Stranger rents a room at the hotel but declines to give a name to the hotelier. That night, he dreams about a man being brutally whipped. In the morning he returns to the barbershop for a bath. Callie Travers shoots at him after he is in the tub; inexplicably, he remains uninjured.

Thomas Dunson (John Wayne) is a stubborn man who wants nothing more than to start up a successful cattle ranch in Texas. In the time of Frederick Barbarossa, in the area of Italy known as Lombardy, Dardo Bartoli (Lancaster) is walking with his son Rudi (Gordon Gebert) when they encounter Count Ulrich (Frank Allenby), known as "the Hawk", together with his niece, Lady Anne (Mayo), and his lover, Dardo's unfaithful wife Francesca (Lynn Baggett).
