Long ago, a drop of sunlight became a golden flower capable of healing illness, decay and injury. For hundreds of years the flower is used by Mother Gothel to retain her youth until it is discovered by soldiers of a nearby kingdom. They use the flower to heal their ailing queen, who soon after gives birth to Princess Rapunzel. Gothel discovers that Rapunzel's hair has the flower's healing properties but that attempting to take just the hair by cutting it destroys its power, so she kidnaps Rapunzel and raises her as her own daughter in an isolated tower. Once a year, the King and Queen release sky lanterns on Rapunzel's birthday, hoping for their daughter's return.

During Christmas 1955, an 11-year-old Hellboy is told a bedtime story by his adoptive father, Trevor Broom, involving an ancient war between man and magical creatures, started by man's greed. After defeat of the magical creatures' forces, the master of the goblin blacksmiths offers to build an indestructible mechanical army for the elven King Balor. Balor is ridden with guilt and forms a truce with the humans: Man will keep his cities and the magical creatures will keep their forests. Nuada does not agree with the truce and leaves in exile. In 1944, the Nazis build a dimensional portal off the coast of Scotland. With the help of Russian mystic Grigori Rasputin, they intend to free the Ogdru Jahad—monstrous entities imprisoned in deep space—to aid them in defeating the Allies. Rasputin knows this will be an apocalyptic event that he believes will create a paradise. He opens the portal with the aid of his disciples, Ilsa von Haupstein and Obersturmbannführer Karl Ruprecht Kroenen, member of the Thule Society and Adolf Hitler's top assassin. An Allied team is sent to destroy the portal, guided by young Trevor Bruttenholm, who is well-versed in the occult. The German team is killed and the portal is destroyed—in the process absorbing Rasputin—while Haupstein and Kroenen escape. The Allied team discovers that an infant demon with a right hand of stone came through the portal; they dub him "Hellboy" and Bruttenholm adopts him.

In Villach in the 13th century, three women are accused of witchcraft by a priest. While one claims to be a witch through persuasion from the church, one doesn't deny it and curses the priest. He orders them hanged and drowned. That done, he urges the guards to pull them back up for a ritual to make sure the so-called witches never come back to life. The guards refuse, claiming they are dead enough. The priest returns late at night to perform the ritual. The third corpse takes on a demonic appearance and kills the priest.

The film begins with a raid by the apelike Wagabu tribe on the early Homo sapiens Ulam tribe, who possess fire in the form of a carefully guarded small flame which they use to start larger bonfires. Obtained from a natural source, the flame is kept in a fabricated bone satchel and must be fed constantly to keep it alive, because the Ulam don't know how to start a fire. Driven out of their home after a bloody battle with the Wagabu, the surviving Ulam escape but are chased into a marsh by a pack of wolves. The Ulam's bald-headed fire tender escapes with the tribe's remaining fire; however, while crossing a marsh, he all but douses the embers, leaving the tribe doomed to die from exposure and starvation. The Ulam elder decides to send three men, Naoh (Everett McGill), Amoukar (Ron Perlman) and Gaw (Nameer El-Kadi), on a quest to find fire.
