A suicidal Hunter "Patch" Adams (Robin Williams) commits himself into a mental institution. Once there, he finds that using humor to help his fellow patients gives him a purpose in life. Because of this he wants to become a medical doctor and two years later enrolls at the Medical College of Virginia (now known as VCU School of Medicine) as the oldest first year student. He questions the school's soulless approach to medical care and clashes with the school's Dean Walcott (Bob Gunton), who believes that doctors must treat patients as patients and not bond with them as people. Because of this and incidents such as setting up a giant pair of legs during an obstetric conference, he is expelled from the medical school, although he is later reinstated due to his methods actually helping patients improve. Adams encourages medical students to work closely with nurses, learn interviewing skills early, and argues that death should be treated with dignity and sometimes even humor.

In 1973, Sam "Ace" Rothstein (Robert De Niro) is a sports handicapper and Mafia associate who is sent to Las Vegas to run the Teamsters Union-funded Tangiers Casino on behalf of the Chicago Outfit. He hires old friend Billy Sherbert (Don Rickles) as his manager. In between, Ace and his friend, mob enforcer and caporegime Nicholas "Nicky" Santoro (Joe Pesci), narrate how the mob bosses control the Teamsters Union, which gives out money for casinos that they own, such as The Tangiers, and how they also drive off rival crews and get rid of cheaters. Ace becomes the Tangiers' de facto boss by taking advantage of lax gaming laws allowing him to work at the casino while his gaming license is still pending. He doubles the casino's profits, which are skimmed by the Mob before the records are reported to income tax agencies. The bosses are impressed with Ace's work and send Nicky to protect Ace and the whole business, along with Nicky's brother Dominick, Nicky's friend and subordinate Frank Marino (Frank Vincent), and the rest of Nicky's soldiers in his crew. Nicky, however, becomes more of a liability than an asset; his criminal activities- which he makes nearly no effort to conceal- and his violent and vicious temper quickly gets him banned by the gaming board from every casino, and his name is placed in the Black Book. In retaliation, Nicky gathers his own crew, opens a jewelry store and restaurant, begins running unsanctioned shakedowns and burglaries, and soon after is considered the mob boss of Vegas.

The quiet coastal town of Midwich in California's Marin County is invaded by an unseen force which leaves ten women mysteriously pregnant. Nine months later, the babies are born simultaneously on one night, though one is stillborn. At first, they all appear to be normal, but it does not take the parents long to realize that their children are anything but normal. As they grow older, the children are shown to have pale skin, white hair, fierce intellect and steely, cobalt eyes. However, they also do not appear to possess a conscience or personalities. The emotionless children display eerie psychic abilities and remarkable powers, which they use with deadly consequences, unleashing a reign of terror. When they actively use their mind-control powers, their irises or their entire eyes glow in different colours, mostly reddish-orange, but also green, yellow, violet, blue or pure white.

Jimmy Woods (Luke Edwards) is a young boy who has suffered from an unnamed, but serious mental disorder ever since his twin sister Jennifer drowned in the Green River two years earlier. He does not interact with anyone, spending most of his time building things out of blocks or boxes, and he always carries his lunch box with him. He is determined to go to "California", at first nearly the only word he can say since the tragedy. The trauma of the drowning and Jimmy's condition have broken up his family; he lives with his mother Christine Bateman and stepfather, while his brothers Corey (Fred Savage) and Nick (Christian Slater) live with their father Sam (Beau Bridges). When Jimmy is put into an institution, Corey breaks him out and runs away with him to California. Christine and her husband hire Putnam (Will Seltzer), a greedy and sleazy runaway-child bounty hunter, to bring back only Jimmy; he competes with Sam and Nick to find the boys, and both groups sabotage each other's efforts, resulting in chaotic confrontations.
