Teenager Mary Cummings (Jena Malone), who has "been born again her whole life," is about to enter her senior year at American Eagle Christian High School near Baltimore. She and her two best friends, Hilary Faye (Mandy Moore) and Veronica (Elizabeth Thai), have formed a girl group called the Christian Jewels. Mary’s "perfect Christian boyfriend" Dean Withers (Chad Faust) tells her, as they’re swimming underwater, that he thinks he's gay. In her shock, Mary hits her head in the pool and has a vision in which Jesus tells her that she must do everything she can to help Dean. Hoping for a sign, Mary goes to a shooting range with Hilary Faye, who has a "spiritual solution for everything" and tells Mary (not knowing about the situation with Dean) that if all else fails, Jesus could still restore their "spiritual and emotional virginity." Believing that Jesus will restore her purity, Mary sacrifices her virginity to have sex with Dean in an attempt to restore his heterosexuality.

The film is divided into 13 vignettes, each prefaced by an aphorism. Set in New York City, the story revolves around ambitious district attorney Troy, who is stricken with guilt following a hit and run accident in which he injures Beatrice, an idealistic cleaning woman who, forced to reassess her life during her recuperation, finds herself thinking more like her cynical co-worker Dorrie. Midlevel insurance claims manager Gene, unable to cope with his son's downward spiral into drug addiction, is rankled by an unrelentingly cheerful staff member and suffers pangs of regret after firing him without just cause. Craig Schwartz (Cusack) is an unemployed puppeteer in a forlorn marriage with his pet-obsessed wife Lotte (Diaz). Gaining a file clerk job through Dr. Lester (Bean) at LesterCorp, in the strange Floor 7½ low-ceiling offices of the Mertin-Flemmer Building in New York City, he develops an attraction to his coworker Maxine (Keener), who does not return his affections. Craig enters a small door hidden behind a filing cabinet and finds himself in the mind of actor John Malkovich. Craig is able to observe and sense whatever Malkovich does for fifteen minutes before he is ejected and dropped into a ditch near the New Jersey Turnpike. Mark Hunter (Slater), a high school student in a sleepy suburb of Phoenix, Arizona, starts an FM pirate radio station that broadcasts from the basement of his parents' house.
