In 1462, Vlad Dracula, a member of the Order of the Dragon, returns from a victory against the Turks to find his wife, Elisabeta, has committed suicide after receiving a false report of his death. Enraged that his wife is now damned for committing suicide, Dracula desecrates his chapel and renounces God, declaring that he will rise from the grave to avenge Elisabeta with all the powers of darkness. In a fit of rage, he stabs the chapel's stone cross with his sword and drinks the blood which pours out of it.

Five years after saving New York City from the demi-god Gozer, the Ghostbusters—Egon Spengler, Ray Stantz, Peter Venkman, and Winston Zeddemore—have gone their separate ways after having been sued by the city for property damage and barred from investigating the supernatural, forcing them out of business. Ray owns an occult bookstore and works as an unpopular children's entertainer with Winston, Egon works in a laboratory on the Columbia University campus, conducting experiments into human emotion, and Peter hosts a pseudo-psychic television show. In 1979 a priest at the Vatican sees a comet arching over the moon (described as the "Eye of God"), heralding the birth of one chosen to be the mother of Satan's child. The priest is sent on a mission by the pope (Mark Margolis) to find and protect the girl from Satan, although a few Vatican knights (led by a corrupt cardinal (Michael O'Hagan)) insist that she must die. While driving cross-country from Boston to San Diego in their new Jeep Grand Cherokee, Jeff Taylor and his wife Amy narrowly miss colliding with a beat-up Ford F-150 that darts in front of them from a connecting road on a deserted highway. Later, while stopped at a gas station, the truck's driver, Earl, exchanges hostile words with Jeff before the couple resume their journey. Shortly afterwards, their Jeep breaks down in the middle of the desert. Leaving Jeff with the car, Amy accepts a ride from a passing big rig trucker named Red Barr driving a Peterbilt 385, to get to a nearby diner and call for help. Jeff eventually discovers that the Jeep's battery connections have been suspiciously tampered with, and, after reconnecting them, drives to the diner, only to discover that no one has seen his wife. When he catches up to Barr on the road and forces him to stop, the trucker claims he has never seen Jeff or his wife before. Jeff hails a passing sheriff, named Boyd, but a brief search of Barr's truck yields no sign of Amy, and he is let go.
