The sequel begins in 1945, during a period extensive American firebombing over major Japanese cities. Desperate to end the war quickly, the government concocts a plan to eliminate major key leaders of each country. With the support of the Japanese government, the Buddhist shaman Kan’nami Kouou (Tetsuro Tamba) plans to curse the leaders of the Allied forces using magic. Unfortunately, the spirits of those civilians who were horribly killed during the fire bombings culminate together and reincarnate Yasunori Kato (Kyusaku Shimada), the demonic Onmyoji who was suppressed 20 years ago. Once again, Kato wants Tokyo to suffer for its crimes and sets off to stop Kouou’s plan from succeeding so that the war will continue and Tokyo will be destroyed. To challenge Kato, Kouou has hired a young man, Yuko Nakamura (Masaya Kato), who has incredible psychic abilities. During the course of the story, Nakamura meets and falls in love with Yukiko Tatsumiya (Kaho Minami), who is now working as a nurse in a War Victims Hospital and still suffering from traumatic memories of abuse as a child by Kato. The young psychic is unable to contend with Kato’s mighty powers and after several battles (the final involving his power being enhanced through artificial means), he is nearly killed. Despite nearing death, Yukiko’s love gives Nakamura enough strength to perform one last trick which successfully destroys Kato’s physical body. Meanwhile, Kouou realizes that Japan is doomed to lose the war. Thus he changes targets, sparing the Allied leaders, and launching a psychic attack against Adolf Hitler which drives Hitler to commit suicide in his bunker. Yukiko prays to Masakado to seal away Kato’s soul forever.

Yoshimi Matsubara (松原 淑美 Matsubara Yoshimi; Hitomi Kuroki), in the midst of a divorce, moves to a run-down apartment with her daughter, Ikuko (松原 郁子 Matsubara Ikuko; Rio Kanno). She enrolls her daughter in a nearby kindergarten and in order to win custody of her daughter, starts working as a proofreader, a job she held years ago before she was married. The ceiling of the apartment has a leak, which worsens on a daily basis. Matsubara complains to the janitor of the apartment, an old man, but the janitor does nothing to fix the leak. She then tries to go to the floor just above her apartment to find out the root of the leak, and discovers that the apartment is locked.

Two neurosurgeons, Etsuko Ōta and her husband, Yukio, watch a 16mm documentary film of a secret experiment with Japanese, Manchu, and Russian subjects concerning the electrification of the temporal lobe of the brain, ending with the subjects projecting a blinding white light. Their children, Miyuki and Kaori, wake up and watch the film despite Etsuko's insistence for them not to. Years later, a group consisting of Miyuki, Kazushi, Takumi, and Rieko, conduct a mass suicide assisted by Etsuko's assistant, Hattori, though it is actually a masquerade for their initiation to a similar neurosurgery experiment conducted by Etsuko. Miyuki wakes up inside a facility to another of Etsuko's assistants, Hisae, who insists that she has died and is currently astral projecting, even presenting her with her corpse as proof. Miyuki and Rieko are later found to have escaped the facility unnoticed.

Jake (the only survivor from the The Grudge 2) is placed in a mental asylum after the events of the second film under the care of Dr. Francine Sullivan, his caretaker. One day Dr. Sullivan leaves Jake locked in his room as he tried to escape several times, as Jake begs her not to saying "she" will get him. While huddling on his bed, he hears Kayako in the room and begins pounding on the door to be let out. Kayako grabs Jake and begins to throw him around the room. Sullivan is quickly called by an officer, as she watches the footage on camera wondering what Jake is doing to himself (as Kayako cannot be seen on the footage). As they race to his room, they find him dead, with every bone in his body broken with his wrist slashed and blood everywhere.
