Brick Bardo (Thom Mathews) is out to avenge the death of his brother, who was set up in a scandal involving illegal fight fixing in a kickboxing circuit and was killed before he could expose the true culprits. Along with his vicious femme fatale Max (Marianne Taylor), he manages to track down four former fighters who were involved in the scandal and had sufficient motive to want his brother dead: janitor and father, Billy Munoz (Benny "The Jet" Urquidez); Connie Angel (Hope Marie Carlton); arrogant and brash Brent Caldwell (Dale Jacoby); and powerhouse, Mike Johnson (Thunderwolf). He has them drugged, kidnapped and brought to an empty fighting arena where he plans to fight them all to the death in front of each other until either one of them kills him or one of them is revealed as his brother's murderer. As Brick engages each of them in one on one combat, it becomes clear that not everything is what it seems and not everyone is as innocent as they claim to be.

The film is about the residents of the small town of Pebbles Court who are the unknowing test subjects for a new variety of "Vimuville" dietary supplement pills that arrive for free in their mailboxes. The pills are designed to produce the ultimate healthy human, but have unexpected side effects including hallucinations and mutations. Despite the attempts made to warn the townsfolk from a previous test subject, who is now undergoing rapid cellular decay, he arrives too late, and crashes his car and is killed by tentacles growing out of his throat. The pills are consumed by the residents, and produce liquefying flesh, elongated tongues, exploding stomachs, exploding penises, imploding heads, monstrous births, tentacles growing out of the face, living mucus, sentient placentas, and other gruesome mutations. Ultimately more and more of the residents of the Pebbles Court mutate or die horrific deaths, until almost every character has been dispatched.

Barbara and Mazzoli share a romantic moment under a capsized canoe but Mazzoli breaks away before anything further happens knowing that his friend Hammer has been “with” her for a while. Later that night Hammer and Barbara elope and Mazzoli is shocked but accepts the marriage.

Christine Day (Jill Schoelen), a young opera singer in modern-day Manhattan, is searching for a unique piece to sing at her next audition. Curious, Christine and Meg do a little digging on Destler, and discover he may have been responsible for many murders and the disappearance of a young female opera singer he was said to have been obsessed with. While Christine is alone, she sings from the tattered parchment and blood seeps from the notes and covers her hands. Shocked, she discovers this to be an illusion when Meg returns. Christine auditions with the piece and during her performance, an accident with a falling sandbag renders her unconscious and shatters a mirror. She awakens in London in 1881, wearing opera clothing. A different version of Meg (Emma Rawson) is also there. Christine turns out to be the understudy to the diva La Carlotta (Stephanie Lawrence), who is both jealous and resentful of Christine’s skill. During this whole time, Erik Destler (Robert Englund) attacks the scene-shifter Joseph (Terence Beesley) with a blade high above the rafters for almost killing Christine with the falling sandbag, and blaming the accident on him.
