The film opens with T.P (Tony Fish), one of the senior counselors at a camp, is telling a scary story around a campfire to the young campers and the other counselors - who consist of Betsy (Gaylen Ross), Ellie (Jan Claire), Dave (Seth Jones), Stacy (Harriet Bass), Bill (Alexander Murphy Jr.), and Max (Carl Fredericks); the late middle-aged head counselor. After T.P. finishes his story, Max begins to tell of a man named Madman Marz, who killed his whole family with an axe was convicted by being hung for his crimes, only to break free and disappear into the woods. Max continues by saying that anyone who says his name will awaken him and cause him to track and kill that person. Richie (Jimmy Steele), a cocky teenager, stands up and screams Marz's name, smashing a rock into his old house. Max, annoyed at Richie, ends the campfire and sends everyone to their cabins, while he goes into town to retrieve supplies. Richie notices Madman Marz up in a tree, and follows him to his house, where he investigates. As Max and Dave try to retrieve a felling axe that is stuck into a log, everyone heads inside their cabins. Chef Dippy (Michael Sulliva) is killed by Marz outside.

In Vietnam, US soldier Andy Brooks is shot by a sniper and falls to the ground. As he dies, he hears his mother's voice calling out, "Andy, you'll come back. You've got to. You promised." A group of twenty high school students partake an elite class gearing them for the college exam. Among them are the rebellious Kang I-na, her timid best friend, Yoon Myong-heo, and the class joker, Kang Hyun, who harbors an affection for I-na. After several strange incidents in which I-na is almost strangled by Cho Beom, forcing him to be taken to a mental facility, and a student being throttled in the restroom, the class is interrupted by a TV showing of the top-ranking student, Min Hye-yeong, being trapped in a glass tank that is slowly filled with water. A voice announces that he will pose questions to the class; should they fail to answer them in a given amount of time, they will all be kidnapped and killed one by one, exemplified when they realized that some of the students have fail to attend the class. Also, the security system have been disabled and should any one them and their two teachers, Hwang Chang-wook and Choi So-yeong, attempt to leave the school, they will be killed nonetheless. Despite their panicked attempts, the class fail to save Hye-yeong.

The film centres on Riley Jones (Shanley Caswell), a suicidal social outcast at Grizzly Lake High School who harbours an unrequited love for ironically named hipster, and friend since childhood, Clapton Davis (Josh Hutcherson). Clapton is, in turn, head over heels for Ione Foster (Spencer Locke), a beautiful self-absorbed cheerleader who was once best friends with Riley. Their friendship collapsed after Ione pursued Clapton, despite knowing of Riley's feelings for him. All three are waiting out their final year of high school, but it's anyone's guess if they'll ever see graduation, as a serial killer known as Cinderhella is on the loose and preying on Grizzly Lake's student body. The principal (Dane Cook) is certain Cinderhella is a disgruntled student at the school and figures he can keep the prom from turning into a bloodbath by putting the likely suspects in all-day (10:00 AM-10:00 PM) detention on the day of the big dance.

"Doozakh-pur" (Hell's Ground) is a town that is notorious for crimes such as robberies and murders, and many lost commuters disappear without a trace. Some protesters are shown demonstrating against the act of polluting the drinking water. In the same scene, some villagers are seen gathering the same water for drinking purposes. Five college students, namely Vicky, Ayesha, Roxy, Simon, and O.J. are on their way to a concert. While in Hell's Ground they make a brief stop so O.J. could throw up near a river after eating spoiled Laddus. While he is vomiting he is bit in the leg by an unknown man. After crying for help and escaping his attacker he rendezvous with his group and they decide to move on. However, they haven't driven too far when they run into a crowd of zombies in the woods, who has turned into the undead presumably because of the contamination in their local water. The group narrowly manages to escape. Later, when the group feels lost, they are left with no choice but to seek help from the local natives living in the isolation of the Jungle. Upon exploration, a strange man dressed in a robe and representing the attire of that of a Fakir agrees to help them. However, after witnessing his strange and obnoxious behavior the group decides to force the man out of the car. When he barricades the car with his body, Vicky drives over him, crushing his body. Eventually the rented van is out of gas, and Vicky decides to get help from a nearby house. Little did he know that the house was being used as a trap to lure visitors so they could be killed and butchered into pieces by a strange character wearing a Burqa. From that point the plot unfolds to reveal the fate of each character.

A band of ninja warriors, led by an Iga Ninja named Yamata and his comrades Jinnai and Nezumi, are assigned to investigate the crash of a mysterious object from the sky. Upon arriving at the crash site, they discover the remains of other ninjas that have been brutally torn apart. Shortly after joining forces with another band of ninjas led by the kunoichi Rin, they encounter a boy whose village was massacred by an unknown assailant. Before they can get any further explanation from him, the ninjas are attacked by the assailants, who reveal themselves to be aliens from another planet. After a grueling battle that takes the lives of several ninjas, Yamata, Rin and Jinnai kill three of the aliens, but one of them retreats and takes Jinnai with it. A cowering Nezumi rushes back to his home village, only to see the villagers massacred by the aliens. He runs for his life, but is cornered by the alien, which swiftly decapitates him as his head lands on a temple post for a crow to feed on.

Al Simmons (Michael Jai White) is a special forces agent and black ops soldier. Jason Wynn (Martin Sheen), head of the covert government agency for which Simmons works, assigns him to infiltrate a biochemical weapons plant in North Korea. A baby anaconda, which was captured at the end of the previous film, is being used for experiments by a man named Peter Reysner from the previous film. Peter creates a hybrid of the blood orchids from Borneo that enabled the anaconda to grow so large and live so long, and creates a serum for cell regeneration. After the test seemingly work for the baby anaconda to regenerate, Peter burns it. When he disappears, the long anaconda escapes from the cage and kills Peter in a mine filled with blood orchids. J.D. Murdoch, a billionaire suffering from bone cancer, sends his assassin Eugene and his team of henchmen mercenaries to find Peter and the serum of Peter created it that can cure him. He also tells them to be careful of Dr. Amanda Hayes, and to kill her if needed. As Dr. Amanda Hayes, the sole survivor of the previous snake attack, and two officers also go in search of Peter, determined to destroy the serum and kill the snake.

Heather (Peta Wilson) is the head-lifeguard on a Malibu beach, alongside her ex-boyfriend Chavez (Warren Christie), Doug (Remi Broadway) and Barb (Sonya Salomaa). Also on the beach are Jenny (Chelan Simmons), a teenage girl who is reluctantly cleaning the beach for community service after she got caught shoplifting, and Bryan (Nicholas G. Cooper), Barb's boyfriend who proposes to her. Meanwhile, a tremor unleashes a group of Living fossil goblin sharks who begin to devour swimmers along the beach. Chavez travels to a nearby house that is undergoing construction and meets with the workers Colin (Jeffery Gannon), who is Heather's new boyfriend, George (Mungo McKay), Yancey (Renee Bowen) and Karl (Evert McQueen). A warning of a tsunami arises and Chavez returns to the beach, saving Heather who had been knocked into the water by a shark after being sent to investigate some people causing trouble in the water. Doug and Barb evacuate the beach, and take shelter in the lifeguard hut with Heather, Chavez, Jenny and Bryan.
