A baby anaconda, which was captured at the end of the previous film, is being used for experiments by a man named Peter Reysner from the previous film. Peter creates a hybrid of the blood orchids from Borneo that enabled the anaconda to grow so large and live so long, and creates a serum for cell regeneration. After the test seemingly work for the baby anaconda to regenerate, Peter burns it. When he disappears, the long anaconda escapes from the cage and kills Peter in a mine filled with blood orchids. J.D. Murdoch, a billionaire suffering from bone cancer, sends his assassin Eugene and his team of henchmen mercenaries to find Peter and the serum of Peter created it that can cure him. He also tells them to be careful of Dr. Amanda Hayes, and to kill her if needed. As Dr. Amanda Hayes, the sole survivor of the previous snake attack, and two officers also go in search of Peter, determined to destroy the serum and kill the snake.

The film opens a year after the previous film with Charles Garrison (Peter Coyote) arriving at a mortuary with one of the canisters of Trioxin that he salvaged from the previous movie. He is greeted by a team of Russian government officials whose goal is to destroy the last of the canisters to avoid another incident like that of what happened in Necropolis. The film's plot, which develops as a flashback narrated by Dumitriu's young son, opens with what American film magazine Variety called "a mad gallop, with the camera in the saddle, giving viewers a crash course in regional rivalries circa 1925." In the opening scenes, Romanian authorities are shown to be frantically engaged in shutting down a brothel, whose presence they believe would embarrass local high society at a time when a grand ball is set to take place. This scandalized prostitutes include the Hungarian Erzsi, who is also a communist sympathizer, and who irritates the officials by shouting out insults and mooning them through a window. During the latter scene, John Simon notes, Land Forces officers are shown staring up "in mixed horror and admiration at the familiar globe whose owner they promptly identify." As she is beaten up by the soldiers, Erzsi continues to defy her aggressors by shouting up revolutionary slogans coined under the Hungarian Soviet Republic.
