It has two sequels, Basket Case 2 (1990) and Basket Case 3: The Progeny (1991) by the same director. It is notable for its low budget, bizarre humor, and strong violence. The film gained an audience in the 1980s due to the advent of home video and has become a cult film. Kevin Van Hentenryck stars as a normal-looking person who seeks vengeance for the unwanted surgery that separated him from his deformed Siamese twin brother.
Synopsis
Duane Bradley arrives in New York City with a locked basket. After he gets a room at a cheap hotel, the contents of the basket are finally revealed: in it lives his deformed Siamese twin brother, Belial. Although conjoined at birth, the twins were surgically separated at an early age against their will, and Belial deeply resents being cut off from his normal-looking brother. As the twins seek revenge against the doctors responsible for their separation, Duane befriends a nurse, Sharon. Jealous, Belial attacks and kills her when he becomes frustrated with his inability to rape her. Enraged at his brother for his actions, Duane attempts to kill Belial, which results in the two brothers falling from a hotel window.Actors

Brian begins an unwilling symbiotic relationship with a malevolent leech-like brain-eating parasite called "Aylmer". Aylmer secretes a highly addictive, hallucinogenic blue fluid into Brian's brain. In return for a steady supply of the fluid, Brian must seek out human victims for Aylmer, so that he can devour their brains. After his attractive fiancée is cut into pieces in a freak accident involving a lawnmower, aspiring mad scientist Jeffrey Franken is determined to put her back together again. Having survived their fall at the end of the first film, Duane Bradley and his hideously deformed brother Belial are rescued from the hospital by an elderly woman named Ruth who, along with her beautiful granddaughter, are the caretakers of an extended family of similarly deformed individuals. After crudely trying to sew his monstrous former conjoined twin Belial back on to his side at the end of the previous film, Duane Bradley is re-separated from Belial and put in a straitjacket and padded cell in Granny Ruth's haven for "unique (deformed) individuals". After several months of captivity Duane is released by Granny Ruth, who is preparing to take everyone on a road trip to the home of her ex-husband Doctor Hal Rockwell, who will help in giving birth to Belial's equally misshapen girlfriend Eve's babies; before leaving for the trip, Granny Ruth sternly tells Duane to stay away from Belial, who has stopped speaking to Duane telepathically after Duane's attempt to put them back together.

Three high school sophomores, Ben (Tye Sheridan), Carter (Logan Miller), and Augie (Joey Morgan), are trying to recruit new members for their scout group, led by their Scout Leader Rogers (David Koechner). While Augie has always been happy about being a scout, Ben and Carter have second thoughts. Carter tries convincing Ben to quit but he can't as Augie is about to get his Condor Patch. Their conversation of quitting is cut short when Ben accidentally hits a deer. They manage to get help from Carter's sister, Kendall (Halston Sage), and her friends. Ben, who's had a crush on Kendall, gets some advice from her and they all see that the dead deer has vanished. Before they leave, the two are invited to the "Secret Seniors Party" and Carter is given the address. When they go to get alcohol for the party, they meet Denise (Sarah Dumont), a cocktail waitress that works at a strip club. She and Ben hit it off and she agrees to buy the alcohol for them. The two then meet up with Augie in the woods so they can set up their campsite. That night, still concerned about the disappearance of Scout Leader Rogers, the trio talk about how great their friendship has been before heading to sleep. Before the night is out, Carter wakes Ben so they can attend the party. Augie catches up, however, and is disappointed by their choice. The two drive into town to find that the bouncer for the strip club is no longer there and they sneak in. The two are immediately attacked by a zombified bouncer and stripper before being saved by a shotgun wielding Denise. Augie goes to Scout Leader Rogers' house to see if he's OK, only to be attacked by him. He quickly subdues his zombie leader by setting him on fire and tying him up.

The story focuses on a theatre troupe, led by Alan (Alan Ormsby). He is a mean-spirited director, who travels with the others by boat to a small island that is mainly used as a cemetery for deranged criminals, to have a night of fun and games. Once on the island Alan tells his group, which he refers to as his "children"— numerous stories relating to the island's history and buried inhabitants. he leads them to a cottage where they are supposed to spend the night. He then opens a chest they had brought with them, puts on a mystical robe and says that they are to prepare for the summation at midnight. Alan takes sheer delight in torturing his cast with threats of firing them if they do not do as he pleases which always makes them go along with his plan. At midnight using a grimoire, Alan begins a séance to raise the dead after digging up the body of a man named Orville Dunworth (Seth Sklarey). Though the original intent of the ritual may have been solely as a joke, Alan appears disappointed that nothing happens.
