Synopsis
Having survived their fall at the end of the first film, Duane Bradley and his hideously deformed brother Belial are rescued from the hospital by an elderly woman named Ruth who, along with her beautiful granddaughter, are the caretakers of an extended family of similarly deformed individuals. When a snooping tabloid reporter and a sleazy photographer threaten to endanger the community's welfare, the two brothers join with the freaks to defend their privacy with a vengeance.Actors

After crudely trying to sew his monstrous former conjoined twin Belial back on to his side at the end of the previous film, Duane Bradley is re-separated from Belial and put in a straitjacket and padded cell in Granny Ruth's haven for "unique (deformed) individuals". After several months of captivity Duane is released by Granny Ruth, who is preparing to take everyone on a road trip to the home of her ex-husband Doctor Hal Rockwell, who will help in giving birth to Belial's equally misshapen girlfriend Eve's babies; before leaving for the trip, Granny Ruth sternly tells Duane to stay away from Belial, who has stopped speaking to Duane telepathically after Duane's attempt to put them back together.

Duane Bradley arrives in New York City with a locked basket. After he gets a room at a cheap hotel, the contents of the basket are finally revealed: in it lives his deformed Siamese twin brother, Belial. Although conjoined at birth, the twins were surgically separated at an early age against their will, and Belial deeply resents being cut off from his normal-looking brother. As the twins seek revenge against the doctors responsible for their separation, Duane befriends a nurse, Sharon. Jealous, Belial attacks and kills her when he becomes frustrated with his inability to rape her. Enraged at his brother for his actions, Duane attempts to kill Belial, which results in the two brothers falling from a hotel window.

Brian begins an unwilling symbiotic relationship with a malevolent leech-like brain-eating parasite called "Aylmer". Aylmer secretes a highly addictive, hallucinogenic blue fluid into Brian's brain. Dr. Edward Pretorius (Ted Sorel) is a scientist who has developed the Resonator, a machine which allows whoever is within range to see beyond normal perceptible reality. His assistant, Dr. Crawford Tillinghast (Jeffrey Combs), activates the machine and soon sees strange creatures in the air. When he is bitten by one of them, he urges Pretorius to turn the machine off. However, the crazed Pretorius refuses. Events transpire to the point where Crawford panics, fleeing outside. When the police arrive, they find Pretorius decapitated. Crawford is subsequently arrested and accused of murder.

The story begins with a military truck transporting barrels of Trioxin. The soldier, driving the vehicle through a downpour, is unaware when a barrel breaks loose and falls into a river. The next morning, a young boy, Jesse Wilson, is at the cemetery with 2 local bullies. The trio investigate the Trioxin tank that they find, and Jesse warns them that they should not tamper with it. The bullies trap Jesse in a derelict mausoleum and leave him. They then return to the trioxin tank and manage to release the toxic gas. A van pulls up to the graveyard, introducing the characters Ed, Joey, and Brenda. Ed explains to Joey that they are there to rob graves; Brenda expresses her fears for cemeteries, but Joey assures her that it will be worth their time and leaves Brenda in the van. He heads into the cemetery with Ed. They decide to loot the mausoleum and open the locked doors, releasing Jesse, who immediately runs home. At his home, Jesse watches his older sister, Lucy, doing aerobic exercises to a work-out video; she tells Jesse to do his homework or he will be grounded.
