In this fourth installment of the erotic mystery-thriller series and set once again in the fictional south Florida city of Blue Bay, Carson Wheetly (Ashley Parker Angel) is the wealthy and spoiled son of millionaire NASCAR car racer Ted Wheetly (Cameron Daddo). Carson hates his father who he thinks might have murdered his mother years earlier in order to inherit her money. Ted's lawyer, George Stuben (Ethan S. Smith), suddenly announces during the reading of the will that Carson cannot inherit Ted's money and estate until he marries. Carson quickly gets married to Rachel, and inherits. Carson and Rachel decide to kill the two other women and keep all the money for themselves. However, Rachel, along with Brandi and Linda, are also plotting to kill Carson so that Rachel can inherit as a grieving widow and split the money three ways. Rachel does murder Carson and inherits his money. Rather than split the money, Rachel plots to murder Brandi, as well as Linda, to keep all of the inheritance money for herself. Brandi, anticipating that Rachel might do just that, plots a scheme of her own to kill Rachel. However, Linda has an agenda of her own to steal the money for herself, along with her shadowy partner-in-crime who is the mastermind behind everything.

Gerard Reve (Jeroen Krabbé), an alcoholic, bisexual novelist, leaves Amsterdam to deliver a lecture at the Vlissingen Literary Society. There, he becomes sexually involved with its attractive treasurer, Christine Halslag (Renée Soutendijk), who is alternately described as a witch, black widow, Delilah and the Devil. The Virgin Mary appears to him in visions to show that he is targeted as her fourth victim. Mary says, "Anyone given a warning must listen to it." Gerard listens and his life is spared. He passes on the warning to Herman, Christine's other lover, who ignores it, thinking that Gerard is trying to scare him off so that he can have Christine for himself. The movie ends with Herman's death, Christine's selection of a fifth victim and Gerard's future uncertain.

Miriam Blaylock (Catherine Deneuve) is a beautiful and dangerous immortal vampire, promising specially chosen humans eternal life as her vampire lovers. Diego Montes (Nacho Martínez) is a bullfighter, forced into early retirement by a goring, and in the opening scene he finds sexual gratification viewing videos of violent death. In retirement he teaches bullfighting, and among his students is Ángel, a diffident young man who suffers from vertigo. During one episode of vertigo in the practice ring, the scene cuts from Ángel to a woman killing a man during sex with a pin she was wearing in her hair, striking at the base of the neck between the shoulder blades, as a matador kills a bull. After class, Diego asks Ángel if he is homosexual, noting that he is not experienced with women. Ángel says he is not and vows to prove himself. Later that day, Ángel attempts to rape Eva (Eva Cobo), a model who is both his neighbour and Diego's girlfriend. As she is leaving him, she trips in the mud and gashes her cheek. At the sight of her blood, Ángel faints.
