Philip Lamont, a priest struggling with his faith, attempts to exorcise a possessed South American girl who claims to "heal the sick". However, the exorcism goes wrong and a lit candle sets fire to the girl's dress, killing her. Afterwards, Lamont is assigned by the Cardinal to investigate the death of Father Lankester Merrin, who had been killed four years prior in the course of exorcising the Assyrian demon Pazuzu from Regan MacNeil. The Cardinal informs Lamont (who has had some experience at exorcism, and has been exposed to Merrin's teachings) that Merrin is up on posthumous heresy charges due to his controversial writings. Apparently, Church authorities are trying to modernize and do not want to acknowledge that Satan as an actual evil entity exists.

Robert Thorn, an American diplomat stationed in Italy, is told that his son died during birth. Unknown to his unconscious wife, Katherine, Robert adopts an orphaned newborn at the suggestion of the hospital's Catholic priest, Father Spiletto. The story follows music composer/singer Winslow Leach, seen by satanic record producer Swan during his performance as a back-up to the 1950s-style nostalgia band The Juicy Fruits, which Swan produces. Swan believes Winslow's music perfect to open "The Paradise" – Swan's highly anticipated new concert hall – and has his right-hand man Arnold Philbin steal it. One month later Winslow goes to Swan's Death Records to inquire about his music but he is thrown out. He sneaks into Swan's private mansion, and observes several women rehearsing his music for an audition. One is Phoenix, an aspiring singer, whom Winslow deems perfect for his music and the two quickly fall for one another. Winslow realizes Swan's plan to open the Paradise with his music. Winslow sneaks in and Swan orders his minions to beat up Winslow and frame him for drug dealing.

During Christmas 1955, an 11-year-old Hellboy is told a bedtime story by his adoptive father, Trevor Broom, involving an ancient war between man and magical creatures, started by man's greed. After defeat of the magical creatures' forces, the master of the goblin blacksmiths offers to build an indestructible mechanical army for the elven King Balor. Encouraged by his son Prince Nuada, Balor orders the building of this Golden Army. The humans are devastated by the army. Balor is ridden with guilt and forms a truce with the humans: Man will keep his cities and the magical creatures will keep their forests. Nuada does not agree with the truce and leaves in exile. The magical crown controlling the army is broken into three pieces, one going to the humans and the other two kept by the elves.

An artist, John Jaspers (Mark Frost) sells his soul to the mysterious M (as for Mephistopheles) (Andrew Divoff) in order to avenge the death of his girlfriend.
