While in post-production on a low-budget exploitation film, Philadelphia sound technician Jack Terry (Travolta) is told by his producer that he needs a more realistic sounding scream and better wind effects. After leaving the studio to record potential sound effects at a local park, he sees a car careen off the road and plunge into a nearby creek. Jack dives into the water to help, discovering a dead man and a young woman, still alive, trapped inside the submerged car. He pulls her to safety and accompanies her to a local hospital. Jack learns that the driver of the car was the governor (and a presidential hopeful); the girl was an escort named Sally (Allen). Associates of the governor attempt to whitewash the incident by concealing that Sally was in the car, and they convince Jack to smuggle Sally out of the hospital with him.

The plot begins with the long-time friendship of three women, Abbey (Nancy Hendrickson), Jackie (Deborah Luce), and Trina (Tiana Pierce), who reunite every year to take a camping trip. Once while setting up their vacation in the woods, they find their trip turns into their worst nightmare when they are captured by a group of two partially insane punk/"hillbilly" hybrids: Ike (Holden McGuire) and Addley (Billy Ray McQuade). The punks lead a comfortable life, living along with their mentally abnormal mother (Rose Ross) in an occult hovel situated amidst the wood. All through the movie, their mother goads her sons into acts of rape, violence, and murder. Eventually one of the women is severely brutalized by Ike and Addley, and the remaining two escape before the first dies from her sustained injuries. They soon regroup, arm themselves, and set out for bloody revenge against Ike, Addley, and Mother. After the girls take their revenge at the end of the film, as they are about to leave the woods they are attacked by the mother's deformed sister, Queenie.

In The Man Who Wouldn't Die art imitates life for Thomas Grace (Roger Moore), a famous British ex-novelist who wrote a series of acclaimed crime stories featuring a villain patterned after real-life criminal Bernard Drake (Malcolm McDowell). Now living in America, Grace works as a hack reporter for a city newspaper. After reading one of Grace's books, Jessie Gallardo (Nancy Allen), a waitress with burgeoning psychic abilities, predicts several murders—with Grace as one of the victims. Her visions identify the enemy as Drake, who has escaped from prison by faking his own death in a fire. As Drake sets out to frame the author for a series of grisly murders based around his novels, it is up to Thomas and Jessie to clear their names, stop him, and stay alive in the process.
