Luca Di Angelo (Fabio Testi) is a smuggler, one member of an organized team trafficking cigarettes and booze up and down the coast off Naples, Italy. After a run-in with the police in which the smugglers manage to get away by faking a boat explosion resulting in the police motorboats responding to the false emergency allowing the smugglers to get away, Luca and his brother Mickey suspect Scherino (Ferdinand Murolo), the head of a rival gang of smugglers, of passing on their actives. Lucia and Mickey take their accusations to their boss Perlante (Saverio Marconi) a sleazy playboy withy numerous Mafia connections, who agrees to look into it. After a nighttime fire at Mickey's racing stables kills a valued racehorse, he and Luca drive over to inspect the damage. But on the way, they are stopped at a fake police roadblock where the assassins dressed as policemen trick Mickey into getting out of the car and machine-gun him to death over and over again (a homage to Sonny Corelone's death scene in The Godfather), while Luca barely escapes injury by hiding on the floor of the car.

In Marseille, an undercover detective is following Alain Charnier (Fernando Rey), a wealthy French criminal who runs the largest heroin-smuggling syndicate in the world. Dr. Rex Martin is a top neurosurgeon, who is active in studying brain malfunctions that cause mental illnesses. High school friend Jim Reston, a successful businessman at Eunice, requires Martin's aid in reaching the mind of John Halsey, a former genius mathematician who once worked for the company and is now a paranoid psychotic at a nearby asylum. Dr. Martin's surgery is intended to successfully alter the patient's mental attitude, either unlocking the corporate secrets within Halsey's brain or else leaving Halsey unable to accidentally share them with anyone else. As Martin begins the surgical procedure, he starts to experience the same paranoid dreams as Halsey. The episodes grow in intensity until it becomes unclear whether Martin is a doctor imagining he's the patient, or a mental patient who succumbed to the delusion that he was a brain surgeon. Martin floats further and further from reality, caught between his loyalty to his business colleagues and his own humanity.

Chicago DEA agent John Hatcher returns from Colombia, where drug dealers killed his partner Chico. As a result of Chico's death and years of dead end work, John retires and heads to his family's home in suburban Chicago. John and his army buddy Max reunite and celebrate at a bar. A gunfight breaks out between local drug dealers and a Jamaican gang called the Jamaican Posse, whose leader is a drug kingpin known as Screwface. John arrests one of Screwface's henchmen as the gunfight ends. The next day, Screwface shoots up the house where John, his sister Melissa, and Melissa's 12-year-old daughter Tracey live.
