Police psychologists review recordings of a man (Robert De Niro), who states his name as Detective David Fisk, the "Poetry Boy" killer. The Poetry Boy earned the moniker for his modus operandi of murdering criminals and leaving short poems with their bodies. Fisk reveals that he looks up to his partner of almost 30 years, Tom Cowan (who the audience is led to believe is the character portrayed by Al Pacino), and considers him to be his role model of how a cop should be. As he investigates the missing person report of a teenager named Chris Gant (based on Gacy's genuine final victim, Robert Piest), Des Plaines, IL detective Lt. The film begins with Emil Slovak (Karel Roden) and Oleg Razgul (Oleg Taktarov) arriving in America. They were former criminals and after getting out of prison have come to the U.S. to claim their part of a bank heist in Russia (or somewhere in Czech Republic). Within minutes of arriving, Oleg steals a video camera. They go to the run down apartment of their old partner and demand their share. He doesn't have it so Emil stabs him and his wife to death, as Oleg tapes it with the camera. The couple's friend, Czech immigrant Daphne Handlova (Vera Farmiga), covertly witnesses the murders from the bathroom, but she escapes before they can get her too. To hide the crime, Emil burns down the apartment.

In 1991, Thomas Hardy (Bruce Willis), a Pittsburgh Police homicide detective, has just broken the Blue Code of Silence by informing on his partner and cousin, Jimmy Detillo (Robert Pastorelli), for using excessive force on a suspect. He attends the Policemen's Ball with his father, Vince Hardy (John Mahoney). However, the ball is postponed after a police scan goes out indicating a serial killer nicknamed the Polish Hill Strangler has been spotted on the 31st Street Bridge. While pursuing the killer, Tom gets into an accident and is knocked unconscious. When he regains consciousness, he learns his father has been shot dead and the killer has escaped. Police make an arrest based on the testimony of Chick Chicanis (Gareth Williams). Meanwhile, rather than go to prison, Jimmy climbs to the top of the 31st Street Bridge and jumps off. His body is never found.
