A group of vampires look for the hidden resting place of Drake, also known as Dracula—the genetic "father" of their species. They find him in a Syrian ziggurat tomb, although not before, in a moment of blood lust, he kills one of them. The vampires capitalize on Blade's reputation as a serial killer and frame him for the killing of a familiar. At Dark Waters, a Native American sacred land containing an enigmatic swamp spirit, a teenager is murdered by a plant-like monster. The following day, young replacement sheriff Kyle Williams reaches Bywater and meets with deputy sheriff Fraser, who tells him the previous sheriff is among 47 missing persons since oil tycoon Fred Schist bought the ancient tribal lands from shaman and Seminole chieftain Ted Sallis, the first to disappear. Schist claimed Sallis had sold legally and escaped with the money, and asked the sheriff for help: Local protestors opposed his perfectly legal activities, and mestizo scoundrel Renee Laroque was sabotaging his facilities. Williams investigated this while trying to find an explanation for the missing people, some of which were found brutally murdered with plants growing from inside their bodies. Photographer Mike Ploog and shaman Pete Horn tell Williams local legends about the guardian spirit, suggesting that it could be real.

During Christmas 1955, an 11-year-old Hellboy is told a bedtime story by his adoptive father, Trevor Broom, involving an ancient war between man and magical creatures, started by man's greed. After defeat of the magical creatures' forces, the master of the goblin blacksmiths offers to build an indestructible mechanical army for the elven King Balor. Encouraged by his son Prince Nuada, Balor orders the building of this Golden Army. The humans are devastated by the army. Balor is ridden with guilt and forms a truce with the humans: Man will keep his cities and the magical creatures will keep their forests. Nuada does not agree with the truce and leaves in exile. The magical crown controlling the army is broken into three pieces, one going to the humans and the other two kept by the elves.

Al Simmons (Michael Jai White) is a special forces agent and black ops soldier. Artist and graphics designer Patience Phillips (Halle Berry) is a meek people pleaser whose only support is her best friend Sally (Alex Borstein). She works for a cosmetics company called Hedare Beauty, which is ready to ship a new skin cream called Beau-Line, that is able to reverse the effects of aging. However, as Patience visits the factory where it is being manufactured, she overhears a discussion between the scientist, Dr. Ivan Slavicky (Peter Wingfield), and Laurel Hedare (Sharon Stone), the wife of company owner George Hedare (Lambert Wilson); speaking about the dangerous side effects from continued use of the product. Laurel's guards discover Patience and are ordered to dispose of her. Patience tries to escape using a conduit pipe, but the minions have it sealed and flush her out of it, drowning her. Lying washed up on the shore and lifeless, Patience is mysteriously brought back to life by an Egyptian Mau cat, who appeared at her apartment earlier, and from that moment on develops cat-like abilities.
