The film begins with the conclusion of the previous film after Michael Myers (Don Shanks) fell down a mine shaft. The state troopers tossed dynamite down the mine to make sure that Michael was dead. But Michael escapes and stumbles into a nearby river, where he is found by a hermit. Michael falls into a coma for one year while in the hermit's care. On October 30, 1989, Michael awakens, kills the hermit, and returns to terrorize Haddonfield, where his young niece Jamie Lloyd (Danielle Harris) continues to live after nearly being killed by Michael the year before.

Ray "Elvis" Patterson (William Forsythe), an ex-con, joins forces with a sexy, high-priced call girl named Rita (Kari Wührer) to help clear his name from a murder charge 14 years earlier while a brutal gangster is after him for the location of some stolen money. Virginia congressman Gene York (Michael Woods) and his attorney wife Karen (Faye Grant), after numerous failed attempts to have children, go to a nun-owned orphanage where they adopt a child that they name Delia from a nun named Sister Yvonne. At first, other than Delia scratching Karen and the later heart attack of the preacher overseeing her baptism, all seems normal. But seven years later, after the Yorks adopt a rottweiler that they name Ryder, Delia (Asia Vieira) starts to display the traits and personality of an increasingly violent and manipulative sociopath. Furthermore, as the family doctor Hastings reveals that she is going through puberty, strange events begin to occur around Delia, including the death of the father of a boy whom she terrorized. Jo Thueson, a New Age practitioner hired by the Yorks as a nanny to help while Gene runs for the Senate, senses something suspicious about Delia after finding her healing crystals blackened by the girl's touch.

Pendant la Deuxième guerre mondiale, en France. In this tragic tale of misunderstanding, obsession, and increasing madness, "Elle," a beautiful young woman (Isabelle Adjani) settles into a small town in the south of France with her introverted mother (Maria Machado) and physically handicapped father and soon becomes the subject of wild speculation because of her aloofness and at the same time, her obvious sexuality. The young woman is actually caught up in the desire to avenge the long-ago rape of her mother, a rape committed by three men who had arrived at her isolated house in a van which contained an old piano which they were delivering. A shy car mechanic (Alain Souchon) becomes enamored of her, and the woman suddenly sees him in a different light when she learns that his father, now dead, was an Italian immigrant who had owned and tried unsuccessfully to pawn the piano. Intent on taking action against the mechanic's family to right the wrong suffered by her mother, the daughter begins to lose her grip on sanity when she finds out that the men she suspects of the rape are actually innocent. In fact, her father had long ago exacted his own vengeance on the real culprits. This knowledge pushes her over the edge, and she has to be institutionalized. Meanwhile, the young mechanic misunderstands what happened and that leads to tragedy; he tracks down and kills the innocent men Elle had suspected of raping her mother, believing them to be responsible for Elle's current condition.
