Krishnakumar is orphan and a policeman who lived with his younger sister Sheela. Sheela falls in love with her classmate Babu and Krisnha accepts him as his brother in law. But tragedy strikes the couple on their honeymoon when Babu was beaten into a coma and Sheela was raped and killed. The police arrested four middle-age friends who follow them. But Krishnakumar found out that his sister's murderers were actually Babu's three classmates but now they are freed thanks to M. S. Menon, one of the killer's father. Chandran (Mammootty) is a truck driver and Ravi ( Mukesh), the cleaner of the truck. Chandran is very affectionate towards Ravi, for whom, Chandran is like an elder brother. Chandran spends most of his leisure time in booze and women, while Ravi is busy in saving all the money, as he has got a family. One day, Ravi gets a letter from Remani (Jalaja), his wife saying that there is festival in the village temple set to happen for the coming Friday and she requested him to be there .Excited Ravi sets out to his home with all the cash he has saved as he has plans to build a house and to be there for at least two months. But on his way back home, he was killed in an accident. Chandran arrives at the village of Ravi, with his dead body. He finds out the miserable condition of his family and decides to stay for a few days to financially assist Remani and help her fulfill Ravi's dream house. But during his stay, he gets involved in several social issues. He slowly joins the job of ferrying sand under Kochu Varkey, a rich businessman of the area. But, with time, he enrages Kochu Varkey (Prathapachandran) and his son Sunnykkutty (Vijayaraghavan), invites animosity from them. Rajamma ( Seema), a bold and independent lady who makes a living by running a tea stall falls in love with Chandran, but certain unexpected incidents happen in the life of Chandran that turns everything upside down.

Nirnayam is based on 1993 American action film The Fugitive starring Harrison Ford as lead actor. Dr. Roy (Mohanlal) is an honest surgeon who works in a private hospital. He falls in love with his colleague Dr. Annie (Heera Rajgopal). When Annie discovers a frightening organ-smuggling operation in her hospital, she is murdered. Dr. Roy is framed for the crime due to circumstantial evidence. However, on his way to jail, an accident occurs and he escapes. A fugitive from justice, he tries to discover the truth. He eventually finds aid in a police officer, Javed (Lalu Alex). Eventually, Dr. Roy is able to bring the perpetrators behind the kidney scam to justice and kill Ifti, (Sharat Saxena), his wife's murderer.
