Concert pianist Paul Orlac (Conrad Veidt) loses his hands in a horrible railway accident. His wife Yvonne (Alexandra Sorina) pleads with a surgeon to try and save Orlac’s hands. The surgeon transplants the hands of a recently executed murderer named Vasseur. When Orlac learns this, horror obsesses him. He is tortured by the presence of a knife he finds at his house, just like that used by Vasseur, and the desire to kill. He believes that along with the hands he has acquired the murderer's predisposition to violence. He confronts the surgeon, telling him to remove the hands, but the surgeon tries to convince him that a person’s acts are not governed by hands, but by the head and heart.

In an unnamed dystopian country, two cops, Eros (Ario Bayu) and Hendro Waluyo (August Melasz) investigate a case of five people being burned alive after a woman shouted them as thieves. Meanwhile, a down-on-luck narcoleptic journalist, Janus (Fachry Albar), is late to attend his divorce with Sari (Shanty). He searches the recent news and meets the widow of one of the burned victims, a pregnant woman named Ratih. Sneaking on her conversation with a nurse through a hidden tape recorder, Janus later watches as Ratih is hit by a bus outside the hospital and passes out from his narcolepsy, mysteriously waking up inside the hospital afterwards. Janus and his friend, Soebandi, hear from the tape a Javanese prose quoted by Ratih of something that is in "Bukit Bendonowongso, in front of Candi Tujuh Anak Tangga". When the two part ways, a pale white-skinned creature, Pindoro (Yose Rizal Manua) follows Soebandi and apparently beheads him. Janus is taken for questioning by Eros; after the latter leaves, fellow cop Bambang Sutrisno (Frans Tumbuan) threatens Janus to tell him the contents of the tape.

In 2028, multinational conglomerate OmniCorp revolutionizes warfare with the introduction of robotic peacekeepers capable of maintaining law and order in hot spots such as Iran. One evening at Herrington High School in Ohio, a few teachers, the head football coach and the principal leave after discussions about the school's budget. Having left her keys behind, Principal Drake (Bebe Neuwirth) returns to retrieve them. In her office, she is barred from leaving by Coach Willis (Robert Patrick), who becomes increasingly erratic, then impulsively stakes a pencil through her hand and Drake facially slashes Willis with keys. Drake flees to the main school doors only to find the handles chained up and drama teacher Mrs. Olsen (Piper Laurie) is outside, unable to assist Drake. Willis is then ambushed by Drake hiding in a room, smashing a jar on his head, seizes his keyset and grabs scissors for a weapon. Drake runs to the doors with Willis at her tail, unable to immediately find the correct key allows Willis to get up close and obstruct door closure, but Drake successfully closes the padlock. Olsen then consecutively stabs Drake with the scissors with an emotionless face, and reveals that Olsen is in cahoots with Willis.

In 1981, Dulce (Ruddy Rodríguez) lived in an old house with her sons Leopoldo (Rosmel Bustamante) and Rodrigo (Hector Mercado), and her husband Juan José (Gonzalo Cubero). As there are no other people in the house, Dulce is arrested and imprisoned for the triple murder. Thirty years later, an elderly Dulce has been released from jail—under the requirement that she serve the rest of her sentence under house arrest in the very house where the murders took place. Once settled, Dulce is visited by a local priest (Guillermo Garcia) who wants desperately to get her to turn back to God as Dulce has lost all her faith in religion. She successfully manages to enlist his help in discovering what exactly happened in her home thirty years ago.
