Chev Chelios (Jason Statham) is a Chinese-based British hitman, working for a crime syndicate headed by Uglyman (Carlos Sanz). Chelios is contracted by Carlito to kill mafia boss Don Kim (Keone Young) as members of the Triads have been encroaching on Carlito's business. Carlito betrays Chelios by making it appear he assassinated Don Kim on his own, without his authorization. Ricky Verona (Jose Pablo Cantillo), an ambitious small-time criminal uses the opportunity to conspire with Carlito against Chelios; Verona will kill Chelios so the Triads do not retaliate, and then take Chelios's place as Carlito's new hired gun. While Chelios sleeps in his apartment, Verona, his brother Alex (Jay Xcala), and several henchmen break in and inject Chelios with a Chinese synthetic drug which inhibits the flow of adrenaline, slowing the heart and eventually killing the victim. Chelios wakes to find a recording left by Verona showing what he has done.

Picking up two or three years after where the original left off, narcotics officer Jimmy "Popeye" Doyle (Gene Hackman) is still searching for elusive drug kingpin Alain Charnier (Fernando Rey). Orders from his superiors send Doyle to Marseilles, France, to track down the criminal mastermind and bust his drug ring. Once in France, Doyle is met by Inspector Henri Barthélémy (Bernard Fresson), who resents his rude and crude crimefighting demeanor. Doyle then begins to find himself as a fish out of water in France, where he is matched with a language he cannot understand. Doyle is shown round the police station where he finds his desk is situated directly outside the toilets. He tells Barthélémy that he is not satisfied with this positioning and hopes it is not a joke at his expense. Barthélémy informs Doyle that he has read his personnel file and is aware of his reputation and especially hopes he has not brought a gun with him as it is strictly forbidden in France for visiting police officers from other countries to carry firearms.

Dr. John H. Tom (Steven Mackintosh), a middle class working employee, finds himself at rock bottom after he loses his job and his girlfriend in the same week. Believing that he has no choice but to end it all, Tom ventures into a derelict squat in East London, in an attempt to purchase a gun from homeless crack addict D (Ashley Walters). However, D attempts to extort more than the agreed price, in order to pay off his debt to local drug dealer Hoodwink (Andy Serkis), to whom he owes a significant amount of money. However, the gun is not his to sell - it actually belongs to Hoodwink, who will do anything in his power to get it back. Tom's attempts to reason with Hoodwink leave him with more trouble than he bargained for - and trying to buy him off only gets him seriously abused. Despite trying unsuccessfully to shift the blame onto not-so-macho drug dealer Ray (Adam Deacon), Tom and D gradually get to know what makes each other tick - and digress for reasonable life - as they try to find a way out of the situation. However, with Hoodwink still on the warpath, the day soon becomes a cat and mouse game of survival for the pair.
