The story follows music composer/singer Winslow Leach, seen by satanic record producer Swan during his performance as a back-up to the 1950s-style nostalgia band The Juicy Fruits, which Swan produces. Swan believes Winslow's music perfect to open "The Paradise" – Swan's highly anticipated new concert hall – and has his right-hand man Arnold Philbin steal it. One month later Winslow goes to Swan's Death Records to inquire about his music but he is thrown out. He sneaks into Swan's private mansion, and observes several women rehearsing his music for an audition. One is Phoenix, an aspiring singer, whom Winslow deems perfect for his music and the two quickly fall for one another. Winslow realizes Swan's plan to open the Paradise with his music. Winslow sneaks in and Swan orders his minions to beat up Winslow and frame him for drug dealing.

In a small rural village with an African American population, a church group is holding a riverside baptismal service, and one of the faithful being immersed is the recently married Martha (Cathryn Caviness). However, Martha’s husband Ras (Spencer Williams) is absent from the service – he claims he was hunting, but he actually poached a neighbor’s boar. At home, Ras accidentally shoots Martha when his rifle drops on the floor and discharges. The church congregation gathers at Martha’s bedside to pray for her recovery, and during this period an angel (Rogenia Goldthwaite) arrives to take Martha’s spirit from her body. She is brought to the Crossroads between Heaven and Hell, and initially she is tempted by the slick Judas Green (Frank H. McClennan), who is an agent for Satan (James B. Jones). Judas takes Martha to a nightclub, where the floor show includes an acrobat and a jazz singer. Judas arranges to have Martha employed by the roadhouse owner Rufus Brown, but the angel returns and advises Martha to flee. As she is escaping, a nightclub patron mistakenly believes Martha is a pickpocket who robbed him. A chase ensues and Martha races back to the Crossroads, where Satan (along with a jazz band on a flatbed truck) is waiting for her arrival. The angel appears to protect Martha from the mob, who are driven away. The sign at the Crossroad is transformed into the vision of Jesus Christ being crucified, and Christ’s blood drips down on Martha’s face. She awakens to discover she is home and her health is restored. Martha is reunited with her husband, who has now embraced religion. The angel who took Martha on her journey returns to bless the marriage.

The story commences on the streets of Prague on a grey morning with commuters bustling about. We are introduced to the figure of an Everyman, played by Petr Čepek, a colourless figure emerging from a metro station. On his way home, the man encounters two men handing out flyers. It is a map of the city with a location marked in. He shrugs and discards it, returning to his lodging. As he opens the door, a black cockerel runs out. The man sits down to eat, cutting himself a slice of bread. He discovers an egg concealed inside the loaf. He cracks it open but it is empty. Suddenly the lights go out and the wind rises. Objects are thrown about the room. The commotion ceases; the man goes to the window and looks down to where the two men from earlier are staring up at him. One of them holds the cockerel. The man closes the blind and returns to the table, where he finds the map and, using his own map of the city, traces out the location marked.

Oscar (Nathaniel Brown) lives in Tokyo with his younger sister Linda (Paz de la Huerta) and supports himself by dealing drugs, against the advice of his friend Alex (Cyril Roy), who attempts to turn Oscar's interest toward The Tibetan Book of the Dead, a Buddhist book about the afterlife. Kate (Kacey Barnfield) is traveling around Ireland in an R.V. with her ex-boyfriend Ryan (Oliver James), brother Joel (Colin Maher) and friends Hailey (Eliza Bennett), Chuck (Diarmuid Noyes), Tommy (Kobna Holdbrook-Smith) and Anita (Róisín Murphy). After driving into the countryside, the group stop off at a small shop, where they encounter Luca (Ned Dennehy) who attempts to scam them from buying a medallion, and warns them it is dangerous before Chuck steals the medallion and the group attempt to make a quick get-away. However, as they drive away in their R.V. they hit an old woman, who puts a curse on the group, telling them the mythical bird the roc will take vengeance on them, before dying. The group quickly drive away in their R.V. but hit a patch of thick fog and become lost, as fears of the curse heighten. Stopping the R.V. they encounter a young boy in the road. Anita leaves the R.V. to talk to the boy, but he runs away before the roc grabs Anita and drags her into the air. Her body is dropped in front of the R.V., with half of her face mauled off before the roc returns and flies away with her.

Andy McGee (David Keith) met his future wife, Vicky Tomlinson (Heather Locklear), in college while they were earning money by participating in an experiment in which they were given a dose of a low-grade hallucinogen called LOT-6. The experiment grants Vicky the ability to read minds; Andy can make people do and believe what he wants, but the effort gives him nosebleeds (the novel revealing them to be "pinprick" hemorrhages). Philip Lamont, a priest struggling with his faith, attempts to exorcise a possessed South American girl who claims to "heal the sick". However, the exorcism goes wrong and a lit candle sets fire to the girl's dress, killing her. Afterwards, Lamont is assigned by the Cardinal to investigate the death of Father Lankester Merrin, who had been killed four years prior in the course of exorcising the Assyrian demon Pazuzu from Regan MacNeil. The Cardinal informs Lamont (who has had some experience at exorcism, and has been exposed to Merrin's teachings) that Merrin is up on posthumous heresy charges due to his controversial writings. Apparently, Church authorities are trying to modernize and do not want to acknowledge that Satan as an actual evil entity exists.
