A counterfeiter of money is attacked by two masked men and threatened to inform them where a certain amount of counterfeited money is. The counterfeiter pleads ignorance and is subsequently killed. Tommy 'Hopscotch' Hopkins, who has the money, learns of the death of the counterfeiter and begins to fear for his own safety. Consequently, he asks his friend, the street smart Frankie (who owns a strip club), to assist him in an exchange of the fake money for real money which he has organised with some acquaintances. Frankie reluctantly agrees to assist his friend.

Charlie Wright's (Aidan Quinn) business empire is revealed to be a Ponzi scheme, having taken as much as eleven billion from his clients. Leaving his office after hearing about his punishment, Wright is intercepted by FBI Agent Hobbs (Mario Van Peebles), who is unable to arrest Wright as at the moment there is not a warrant. Stalling time, Hobbs gets his warrant and Hobbs' partner Jimmy (Jordan Belfi) intercepts Wright as he pulls out of the office garage. Looking inside the car, Hobbs is dismayed to find that Wright is gone. The ensuing investigation leaves few clues as to where Wright fled.

The film begins with a shot of Victor Ajax (Reed Birney), who has been sentenced to death, sitting in an electric chair. The film then flashes back to show that Victor once was a promising young technician in the employ of Trend-Odegard Security. Mr. Trend (Edward R. Pressman), co-owner of the company, has learned of a plan by his partner to sell the company to Renaldo "The Heel" (Bruce Campbell) and responds by hiring two exterminators who promise to "kill all sizes" (Brion James and Paul Smith) in order to eliminate Odegard and his plan. When Victor, who has been installing security cameras in Trend's apartment building, seems about to go back to the store, Trend distracts him with a lecture about "the grand design" and sends Victor on a quest to find his dream girl.

Claire Bartel is pregnant with her second child. At a routine checkup she is sexually molested by her new obstetrician, Dr. Victor Mott. Traumatized, she tells her husband, Michael, who encourages her to report him to the police. Later, Dr. Mott's pregnant widow is told by her lawyers that her husband's assets have been frozen because of the lawsuits and that she will lose her luxurious home. Mrs. Mott goes into early labor, loses her baby and, after hemorrhaging, is given an emergency hysterectomy. While recovering in the hospital, she sees a news story featuring Claire as the woman who alerted the police, leading Mrs. Mott to swear vengeance.

Agent Wyler (Lewis Arquette) finds Cash's backup Walter PPK pistol on the floor with a silencer attached and arrests both Cash and Tango. At their murder trial, Tango and Cash are incriminated by an audio tape, secretly given to Wyler by Perret's henchman Requin and verified in court by an audio expert, which appears to reveal them shooting the undercover FBI agent after discussing a drug purchase. They plead no contest to a lesser charge in exchange for reduced sentences in a minimum-security prison, but are instead transported to a maximum-security prison to be housed with many of the criminals they arrested in the past.

A man is seated in a diner who is revealed as Harold, a traveling salesman. He is joined by a gentleman who makes him uncomfortable. After leaving the diner, Harold is found dead, which leads to FBI Agents Thomas Mackelway (Aaron Eckhart) and Fran Kulok (Carrie-Anne Moss) being brought in on the case. There appears to be history between Mackelway and Kulok and a cloud of competency is addressed by flashbacks of Mackelway's recent suspension and issues he's undergone. As the investigation proceeds, the agents begin to become aware of the possible existence of "Suspect Zero", a mythical "super serial killer" responsible for hundreds of deaths across all 50 states who leaves no evidence behind to link his crimes together.
