The film is narrated by Anna, a German woman who tells the story of her parents before, during and after World War II. Her mother, Lene, meets her father, Hans, and they fall in love and marry. As the war breaks out, Hans is conscripted into the army and Lene suffers through air raids in Berlin. Her relationship with Hans becomes increasingly strained during his infrequent returns home on leave. She gives birth to Anna, and undergoes great hardship to keep her safe, crossing a forest in winter to escape to her family in the country, and later living a life of poverty in the ruins of postwar Berlin. As life returns to normal after the war, she and Hans are unable to rekindle their love, and Lene spirals into depression, exacerbated by the affliction of a crippling facial paralysis. Handsome Jeronimo Rugera is hired to tutor the rich heiress, Josephe Asteron. They fall in love, yet the church forbids their relationship, and Josephe is hidden in a convent. In 1944 in the Republic of Salò, the Fascist-occupied portion of Italy, four wealthy men of power, the Duke, the Bishop, the Magistrate, and the President, agree to marry each other's daughters as the first step in a debauched ritual. They recruit four teenage boys to act as guards and four young soldiers (called "studs", "cockmongers", or "fuckers"), who are chosen because of their big penises. They then kidnap nine young men and nine young women and take them to a palace near Salò. In Lagrange, Ohio, Curtis LaForche (Michael Shannon) has apocalyptic dreams, and visual and auditory hallucinations, of rain "like motor oil", swarms of menacing black birds, and being harmed by people close to him; but, he hides all of this from his wife, Samantha (Jessica Chastain), and their deaf daughter Hannah (Tova Stewart). At a government laboratory in rural California, a weaponized version of influenza (called Project Blue) is accidentally released, immediately wiping out everyone on staff except for military policeman Charles Campion and his family, who flee the base. However, Campion is already infected by the superflu, nicknamed "Captain Trips", and spreads it to the outside world. That evening, Campion crashes his car at a gas station in East Texas where Stu Redman (Gary Sinise) and some friends have gathered. When they investigate, they find Campion dying of the flu next to his wife and baby daughter, who are already dead. Campion tells Stu with his dying breath that he was followed from the base by a mysterious figure, and states "You can't outrun the Dark Man". The next day, the U.S. military arrive to quarantine the town. While the other townspeople quickly become ill and die, Stu remains healthy and is confined at a CDC facility in Vermont order to study a possible cure.
