The film begins with a group of divers installing a power cable at the bottom of the Pacific Ocean, when a shark swims up and kills one of them. Six months later, lifeguard Ben Carpenter (from the first Shark Attack film) and his partner Esai drive out to sea to catch some lobster. While diving, Ben finds a broken power cable with a large shark tooth stuck in it. After he pulls out the tooth, he is caught by two other divers, and soon goes back to land. Later he posts a description of the tooth online, but as he cannot find a matching shark tooth on the internet, he names it "mystery shark". Natural history researcher named Cataline "Cat" Stone gets the message, finds him and looks at the tooth. She believes it to come from a huge prehistoric shark called Megalodon (Greek for "big tooth").

On October 30, Devil's Night in Detroit, Sergeant Albrecht is at the scene of a crime where Shelly Webster has been beaten and raped, and her fiancé Eric Draven then died on the street outside, having been stabbed, shot, and thrown out of the window. The mad scientist named Preston King (Jeffrey Combs) invites the doctors that could not save his son Paul King (Anton Argirov) from kidney cancer before to his uncharted island in the western Pacific. There, Tom Reed (William Forsythe), Paul's widowed wife Amelia Lockhart (Hunter Tylo), who believes Paul died five years ago from cancer, and several meet on the island. There, Dr. King reveals that Paul never died and was saved from death by being injected with hammerhead shark DNA and filling his stem cells. This turned Paul into a half-man, half-shark creature. Dr. King leaves and releases the water from the tank where he was keeping Paul and hopes that he will eat them as revenge for them firing him from his job five years ago. As Paul, now known as Hammerhead, chases after them, they escape along with Hammerhead.

After Andy Flynn destroyed the original Sharktopus (a half-shark half-octopus creature), its various pieces floated back out into the ocean. However, among the scattered anatomy was an egg sack and marine biologist Lorena finds herself the ward of the creature's sole offspring which she begins raising at her uncle's aquarium to mixed success. Meanwhile, Dr. Rico Symes is hard at work on his own biological weapon. After harvesting a pterodactyl's DNA, he used the DNA from a barracuda to fill in the gaps, creating a monster that can terrorize land, sea, and air. Things are looking pretty good during the initial test flight until the creature is hijacked by a rogue engineer, looking to sell it the highest bidder. He probably would have gotten away with it if he didn't spill his coffee all over his console, but now the Pteracuda is free to rampage the coast in search of food.
