In alternate moment of the series timeline, set in the near future, Smart Brain has accomplished its goal of world domination — with 90% of Earth's population being Orphnochs while the human race had diminished to near-extinction. Takumi Inui, the user of the Faiz Gear, was regarded as mankind's savior until he was presumed killed during an attack by Smart Brain's Riotroopers. Since then, his friend Mari Sonoda has formed a human resistance group that have staged attacks on Smart Brain's headquarters in an attempt to steal the top-secret "Emperor Belts," but to no avail. The Orphnochs Yuji Kiba, Naoya Kaido and Yuka Osada wish to see both Orphnochs and humans co-exist in peace, but they are not taken kindly by the humans while Smart Brain chief Kyoji Murakami (who is now reduced to a head in a water tank) reminds them that humans no longer have a place on Earth. Murakami then discloses the existence of the Emperor Belts by introducing the Orphnoch Leo, who changes into the powerful Kamen Rider Psyga.

Four years after an alternate ending to the series, in which Blade seals the Joker, the characters have moved on with their lives: Kenzaki is a garbage man which in comparison to the chaos he went through was a huge change of pace, Mutsuki has graduated high school, and Kotarō has published a book about the Kamen Riders to great success, but on Amane's upcoming birthday, it was revealed that she has become a delinquent without the emotional support of Hajime. The Undead have been re-released, and a new trio of Kamen Riders—Glaive, Larc and Lance—have emerged from BOARD, now led by Tachibana. After recapturing two of the Category Aces, Kenzaki and Mutsuki join with the new Riders. When all the Undead are sealed once more, Glaive reveals himself to be the Albino Joker, a white version of the original Joker, and captures Kotarō's niece to obtain the ultimate power, sealing her in the Vanity Card. In order to free Amane from the card, Hajime is unsealed from the Joker Card and the four Riders are again united, intent on saving Amane and stopping the Albino Joker. In the end, Hajime swaps his life for Amane in the Vanity Card and allows himself to be destroyed by Blade to weaken Jashin 14. Blade uses his King Form to cut Jashin 14 in half, putting an end to the Undead.

Set two years after Tiga's final battle, Daigo is approached by a mysterious woman, Kamila, who possessed a dark version of the Spark Lens. It is then revealed that more than 30,000,000 years ago, Tiga was originally evil in nature, part of a group of four that dominated Earth. The group consisted of Dark Tiga, the telepathic Kamila (gold patterns on silver skin), the powerful Dahram (red patterns on silver) and the speedy Hyudra (purple patterns on silver). One day, Tiga fell in love with a local human woman, and decided to convert from darkness to protect her from harm. Being the weakest of the four, Tiga quickly became a target of the group. However, unbeknownst to the group, the original "talentless" Dark Tiga possessed the ability to absorb powers from fallen enemies. As he eliminated each of the three dark members and sealed them away, he absorbed their powers which explains new Tiga's color patterns (purple, red and gold lined with his original silver). Convinced by his ability to convert the last known Spark Lens to good, Daigo accepts the gift and becomes Tiga once again, survives his inner struggles and vanishes the darkness within. Soon after this, he marries Rena Yanase, fellow GUTS member and longtime love interest.(Shin Asuka, Dyna's eventual human host, also makes a cameo during the finale as a junior crew member, in a symbolic passing of the torch moment.

On the run again after the events of the previous TV movie, David Banner is working up north under the name David Belson. Disenchanted and at the end of his rope, David makes his way towards a large city with the hopes of renting a room and staying buried. Unbeknownst to him, the city he arrives in is under the control of a powerful underworld kingpin named Wilson Fisk but is also protected by a mysterious black-clad crimefighter known as Daredevil. When two of Fisk's men come onto the commuter subway train after having committed a jewel robbery, one of them takes an interest in a beautiful woman also riding the train and she rejects him. David witnesses an attempted sexual assault by one of Fisk's men, he transforms into the Hulk and things go haywire. A short while later, David is arrested by the police and wrongfully charged with the crime.

On a distant planet, a wizard named Lerigot is being hunted by Divatox; an intergalactic space pirate, who seeks his golden key in order to traverse an inter-dimensional gateway and enter into matrimony with Maligore, a demon who promises to grant her great riches and power. Hibiki and Asumu are on their way to a Makamou attack on the beach. Once they arrive there, they see dozens of people running from the massive Orochi, a beast rumored to be the strongest Makamou ever created. Once they arrive, Hibiki walks towards the Orochi and transforms, but all of his attacks prove useless against Orochi. The situation is made even more dire when Hibiki jumps in the way of an attack meant for Asumu. Taking the full force of the attack, Hibiki passes out as the Orochi swims away, leaving a panicking Asumu to get help. Hibiki ends up in the hospital and Asumu begins blaming himself for Hibiki's injuries. Feeling useless, he begins helping at Tachibana's, going through dozens of books to try to discover a way to defeat the Orochi. Upon coming across a book with his name on it, Asumu sits down and begins to read what turns out to be the history of Takeshi, the Oni and the Makamou war.

In 1999, an enormous meteor crashed onto Earth carrying extraterrestrial creatures known as Worm within it. These creatures took on the identities of real humans and then killed them. In order to counter this new threat, ZECT was formed, who in turn created the Masked Rider System to combat the Worms. The battles lasts over seven years, depleting Earth's resources and turning it into a barren wasteland.
