In early 1950s Los Angeles, Detective Sergeant Edmund "Ed" Exley (Guy Pearce), the son of a legendary LAPD detective, is determined to live up to his father's reputation. His intelligence, insistence on following regulations and cold demeanor contribute to his isolation from other officers. He exacerbates this resentment by volunteering to testify in a police brutality case, insisting on a promotion to Detective Lieutenant against the advice of Captain Dudley Smith (James Cromwell). Exley's ambition is fueled by the murder of his father by an unknown assailant, to whom he refers as "Rollo Tomasi" to give him personality.

Professor Grady Tripp (Michael Douglas) is a novelist who teaches creative writing at an unnamed Pittsburgh university (the movie was shot chiefly in and around Carnegie Mellon). He is having an affair with the university chancellor, Sara Gaskell (Frances McDormand), whose husband, Walter (Richard Thomas), is the chairman of the English department in which Grady is a professor. Grady's third wife, Emily, has just left him, and he has failed to repeat the grand success of his first novel, published years earlier. He continues to labor on a second novel, but the more he tries to finish it the less able he finds himself to invent a satisfactory ending. The book runs to over two and a half thousand pages and is still far from finished. He spends his free time smoking marijuana.

In the near future war rages across the fictional Republic of Gilead and pollution has rendered 99% of the population sterile. Kate is a woman attempting to emigrate to Canada with her husband and daughter. While attempting to take a dirt road, they are caught by the Gilead Border Guard, who orders them to turn back or they will open fire. Kate's husband uses an automatic rifle to draw the fire, telling Kate to run, but he gets shot, Kate gets captured, whilst their daughter wanders off into the backcountry confused and unaccompanied. The authorities take Kate to a training facility with several other women, where they are all trained to become a Handmaid, a concubine for one of the privileged but barren couples who rule the country's religious fundamentalist regime. Although she resists being indoctrinated into the cult of the Handmaids, mixing Old Testament orthodoxy and misogyny with 12-step gospel and ritualized violence, Kate is soon assigned to the home of the Commander and his cold, inflexible wife, Serena Joy. In the preliminary scenes, set in 1942, Lena (Isabelle Huppert), a pretty 18-year-old, has been arrested and brought to an internment camp for Jews in the Pyrenees. The camp is guarded by members of the French Foreign Legion, and one of them, Michel (Guy Marchand), writes her a note warning her she may be deported to a German Nazi camp, and offers her marriage as a means of escape.
